to Us Pursuant to your notice of expiration dated January 7, 1981 of our special nuclear material license SNM-1620, to expire on March 31, 1981, we hereby submit this ap-plication for timely renewal of that license on this date, as we desire to con-tinue our nuclear powered cardiac pacemaker program when a case clinically re-quires it.
The possession and use of the special nuclear material will be in accordance with the statements, representations, procedures, and physicians responsible, as pre-sented in the original letter of application dated March 11, 1976, which remain essentially the same.
Enclosed herein find the required $40.00 amendment fee pursuant to 10CFR170. Any questions or official correspondence regarding this, or any, license should be specifically addressed to myself at this address in order to avoid loss or delay.
