This refers to the inspection conducted by Mr. James T. Conway of this office on November 17-21, 1986, of your facility at Russellville, Arkansas, and to the discussions of the findings with Mr. William A. McRoberts and members of his staff at the conclusion of the inspection.
The purpose of this inspection was to conduct a programmatic evaluation of the implementation of your QA program in the areas of training, control of special processes, reporting of defects, control of purchased material, nondestructive examination, control of measuring and test equipment, and audits. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements.
Significant items included failures to:
(1) pass on 10 CFR Part 21 requirements to vendors, (2) have current procedures at work stations, (3) complete a final inspection check list, (4) properly maintain measuring equipment, and (5) have complete qualification records for a Level II examiner. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to submit to this office within 30 days from the date of this letter a written statement containing:
(1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.

r Ladish Company February 2, 1987 You are also requested to submit a similar written statement for each item which appears in the enclosed Notice of Nonconformance. In addition, explain the rationale for certifying nuclear material to Section II specifications (e.g.,
SA234 and SA105) on Ladish Company certified material test reports (CMTR) after the fitting / flange has undergone hot forming and heat treat operations at Ladish without performing mechanical testing. The responses requested by this letter are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96 11.
In accord-ance with 10 CFR 2.790 of the Comission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Roon.
