In accordance with requirements of Dravo Corporation's Pipe Fabrication Division Quality Assurance procedures, we wish to report components fabricated and shipped by Dravo which were not in accordance with the code of record under which they were fabricated.
The following describes the circumstances, the evaluation, and the action being taken to correct the components:
1.
On January 15, 1987 it was discovered by Dravo that pipe spools had been shipped to the South Texas Project without magnetic particle or liquid penetrant inspection performed on the 4"
and under butt welds.
These pipe spools were to be fabricated as Class 3 to the ASME III 1974 Edition Winter 1975 Addenda of the code.
The Summer 1975 Addenda added the requirement to magnetic particle or liquid penetrant examine 4"
and under butt welds.
2.
A nonconformance was generated and an investigation begun to determine the extent of this problem.
It was determined that 53 pipe spools had been shipped with this nonconforming condition.
The shipments were made between September and December 1986.
3.
Our customer, Bechtel Energy Corporation, was promptly notified on January 16, 1987 by telephone by our Quality Assurance Manager.
4.
Our General Manager was informed of the nonconformance and that it was under evaluation.
5.
A follow-up letter was written to Bechtel Energy Corporation, South Texas Project, on Janauary 20, 1987 (attached) requesting their help to determine if this should be reported under 10CFR Part 21.

It was determined that 14 of these pipe spools were installed at the South Texas Project and the remainder (39) were in a lay down area.
Arrangements were made to magnetic particle or liquid penetrant the butt welds at South Texas Project on the 14 installed spools and this has been completed.
The remaining 39 are to be returned to Dravo for the required examination.
7.
An investigation followed to determine if a similar condition existed on any nuclear projects in house or shipped during 1986.
This investigation proved negative.
Dravo has not had an answer to our request for evaluation from Bechtel to date but have consulted with Bob Wielgoszinski of Kemper in Chicago.
Both Dravo and Kemper believe this non-conformance described above does not constitute a " substantial safety hazard" reportable under 10CFR21 since this examination was not required on Class 3 systems prior to the Summer 1975 Addenda.

This contract specified fabrication was to be to the 1974 Edition thru Winter 1975 Addenda of the ASME Section III Code.
We have just discovered that our sketches did not specify magnetic parr.icle or liquid penetrant examination for 2-1/2" thru 4" butt welds which was added to the ASME III Code in the Summer 1975 Addenda for Class 3 piping.
The branch welds, however, in every case were magnetic particle or liquid penetrant inspected.
We have identified fifty-three fabricated pipe spools which have been shipped to the jobsite that require this additional NDE c n the butt welds only.
We cannot determine at this time if this is reportable under 10CFR Part 21. Therefore, we need to know the status of these spools and your help to determine if this should be reported under 10CFR Part 21.
