Dear Mr. Johnaon:
This letter is written in response to your letter of January 2, ~1986, which transmitted Inspection Report STN 50-482/85-37. As requested, the violation (50-482/8537-01) identified in the Inspection Report is being addressed in four parts.
(a) The reason for the violation if admitted; (b) The corrective steps which have been taken and the results achieved; (c) Corrective steps which will be taken to avoid further violations; and (d) The date when full compliance will be achieved.

Violation (50-482/8537-01): Nonseismic Glass Level Gauge Not Isolated Fi8' oo Finding:
on CD J 10 CFR Part 50, Appendix B, Criterion V states, in part, " Activities nu affecting quality shall be prescribed by documented instructions,
@O procedures, or drawings of a type appropriate to the circumstances..."

N The Checklist (CKL) EG-120, Revision 5,
" Component Cooling Water System Valve, Switch, and Breake-Lineup," has been established in accordance CD O C with 10 CFR Part 50, Appendix B, Criterion V.
Contrary to the above, CKL EG-120 was inappropriate to the circumstances in that Valves EG-V 160,
- 161, 171, and 172 were opened in accordance with Checklist A of-CKL'EG-120 when the valves should have been closed due to the level gauge between the valves being nonseismic.

This personnel error occurred because the need to isolate ' nonseismic level gauges was not recognized in the procedure preparation / review process.
Corrective Steps Which Have Been Taken And Results Achieved:
L Checklist CKL EG-120 has been revised to show.the four valves ' locked closed.
A review of all safety-related tanks was conducted to identify -similar nonseismic level gauges. Five additional tanks'were identified:
Containment Spray Additive Tank (valves'were closed but not locked)
Emergency Fuel Oil Day Tank A Emergency Fuel Oil Day Tank B Jacket Cooling Water Expansion Tank A Jacket Cooling Water Expansion Tank B The appropriate checklists have been revised to require the level gauge isolation valves locked closed.
All affected valves have been locked closed.
Each has been added to procedure ADM 02-102, Control of Locked Component Status, to ensure proper positioning.
Corrective Steps Which Will Be Taken To Avoid Further Violations:
Corrective actions discussed - above provide adequate assurance that further violations will be avoided.
