CONFIRMATION OF ENFORCEMENT CONFERENCE - HATCH - DOCKET NOS. 50-321 AND 50-366 This confirms the conversation between Mr. H. C. Nix of your staff, and Mr. F. S. Cantrell of my _ staff, on December 30, 1986, concerning an enforcement conference to be conducted at the NRC Region II Office on January 22, 1987, at 9:00 a.m.

We requested this meeting to discuss the design and procedural problems associated with the loss of water from the spent fuel pools in early December.

Enclosed is a proposed meeting agenda identifying specific issues of Concern.

The apparent failure to establish adequate design measures to ensure that the seals in the transfer canal were capable of preventing a significant loss of contaminated fuel pool water.

The apparent failure to follow procedures, lack of adequate procedures, and operation of safety-related equipment without a procedure.

The apparent failure of the failed seals to provide secondary containment integrity with

. respect to leakage of water to the environment.

The possible lack of management controls in this area which contributed to the event.
