- SURRY, DOCKET NOS 50-280 AND 50-281 This refers to the management meeting conducted at your request at the Surry site on January 12, 1987. The meeting was held to discuss your findings and plans concerning the December 9,1986, pipe rupture at Surry. A list of attendees at the meeting is shown in Enclosure 1. Some of the details of the meeting are provided in Enclosure 2.

It is our opinion that this meeting was beneficial in that it enabled us to better understand the event and those actions you plan to take.

In accordance with Section 2.790 of NRC's " Rules of Practice," Part 2. Title 10 Code of Federal Regulations, a copy of this letter and its enclosures will be placed in the NRC Public Document Room.

SURRY PIPE RUPTURE N The topics discussed included the sequence of events, investigative results, industry experience, corrective actions, and recovery plans.

VEPC0 concluded that they understand enough of the failure mechanism involved to take corrective actions and restart the units. The actions include replacing piping that has shown excessive wear and inspecting the suspect areas at certain time intervals to monitor wall thinning so action can be taken to prevent a future rupture. The failure appears to have been caused by the combined effects of the type of material, low oxygen content of the water ,

temperature of the water, fluid velocity and piping geometry, all leading to erosion / corrosion of the pipe. Long term corrective actions include an inspection program, rerouting of piping, and material changes.

VEPC0 will seek concurrence from the NRC prior to restarting the Surry units.
