ON JUNE 7, A HEALTH PHYSICS TECHNICIAN AT FRANK W.
HAKE ASSOCIATES IN MEMPHIS NOTED SMOKE COMING FROM A DUST COLLECTOR OF THE WHEELABRATOR GRIT BLAST UNIT.
THE UNIT WAS SHUTDOWN AND THE FIRE EXTINGUISHED WITHIN TWO HOURS.
THE FIRE WAS CONTAINED WITHIN THE CONFINES OF THE UNIT.
ALL PERSONNEL AND EQUIPMENT WERE SURVEYED AND NO CONTAMINATION WAS DETECTED.
AIR SAMPLES SHOWED NO RELEASE OF RADIOACTIVE MATERIALS TO THE ENVIRONMENT.
THE FIRE WAS CAUSED DURING BLASTING OF ALUMINUM AND MAGNESIUM IN THE WHEELABRATOR.
PERSONNEL HAVE BEEN REINSTRUCTED ON THE HAZARDS OF BLASTING ALUMINUM OR MAGNESIUM IN THE WHEELABRATOR.

ON JUNE 15, A FIRE OCCURRED INSIDE THE TEMPORARY BLAST ENCLOSURE IN THE "D"
SHOP OF FRANK W.
HAKE ASSOCIATES IN MEMPHIS.
THE j
SPRINKLER SYSTEM ACTIVATED AND THE FIRE WAS EXTINGUISHED.
SAMPLES OF THE RUNOFF WATER AND SOIL ARE BEING ANALYZED.
AIR SAMPLES INDICATED NO AIRBORNE RELEASE OF RADIOACTIVE MATERIALS.
THE FIRE APPEARS TO HAVE BEEN CAUSED BY HOT STEEL COMING IN CONTACT WITH THE SIDE OF THE TENT AND IGNITING ALUMINUM OXIDE DUST, NO INDICATION THAT THE TENT MATERIAL WAS FIRE RETARDANT.
THE ASSIGNED FIRE WATCH WAS INATTENTIVE TO DUTIES.
ON JUNE 18, l
OPERATIONS WERE SUSPENDED PENDING A REVIEW OF THE HEALTH PHYSICS PROGRAM AT

ON JUNE 25, AN EMPLOYEE AT GTS JURATEK IN OAK RIDGE WAS INJURED WHEN A B-25 BOX EXPLODED IN THEIR LIQUID VOLUME REDUCTION FACILITY.
THE B-25 BOX CONTAINED USED HEPA FILTERS WHICH HAD BEEN SPRAYED WITH MAGNESIUM OXIDE AND ENVIROBLEND.
