Subject:

Routine Inspection 50-245/87-04; 50-336/87-05 (3/10/87 to 4/13/87)

This transmits the report of the above subject inspection of Millstone Units 1 and

2 by Messrs. T. Rebelowski and G. Grant of this office'. The inspection findings

have been discussed with Messrs. J. Stetz and J. Keenan of your staff.

No violations were identified.

However, Detail 6 of the enclosed report identifies

an operating reactor trip at Millstone 1.

In approximately one year,' this is the

third Unit 1 trip that is attributed to the reactor pressure control system.

The

first two trips were caused by equipment malfunctions in the Mechanical Pressure

Regulator (MPR). The most recent trip was caused by operator error when transfer-

ring control from the MPR to the Electrical Pressure Regulator (EPR).

Please re-

view these challenges to safety systems and inform me of_ your assessment and any

planned corrective measures within 90 days of receipt of this letter.

We also wish to call to your attention that multiple instances of eating while on

duty were observed within the red lines in the Unit 2 control room (Report Detail

3).

Although no inattention to duty was identified, eating within the red-lined

area is considered to have the potential for distracting personnel from their

operating duties. We recommend you review this practice for acceptability.

It

is our view that eating in the control room needs to be carefully controlled to

assure that the location and manner of conduct of the activity hazards neither

operator attentiveness nor the professional atmosphere.
