SUBJECT: INSPECTION OF COMANCHE PEAK DESIGN ADEQUACY PROGRAM (DAP),

IMPLEMENTATION OF DISCIPLINE SPECIFIC ACTION PLAN (DSAP) X, MECHANICAL

SYSTEMS AND COMPONENTS - 50-445/86-18, 50-446/86-15

This letter conveys the results and conclusions nf the subject inspection

conducted by the NRC's Office of Inspection and Enforcement (IE). The inspec-

tion team was composed of NRC personnel from IE as well as consultants. The

inspection took place at the TERA site and Bethesda, Maryland offices during

the periods from May 12 to 16 and 27 to 30, 1986, respectively.

The inspection of DSAP X implementation was the first part of IE's assessment

of TERA's in process reviews. During the period from July 7 to July 17, 1986,

IE performed a second similar inspection for DSAPs VIII, IX, and XI for the' civil /

structural, piping / supports and electrical /I&C disciplines, respectively. A

separate report will be issued for this second inspection. The inspection of

DSAP X addressed two aspects: (1) whether TERA's reviews were representative,

,

such that results of the reviews of specific design documents could be extrapo-

! lated td design documents not reviewed, and (2) whether TERA's reviews were

,

effective in identifying discrepancies.

! Section 1 of the report provides a summary of the results of the inspection and

conclusions reached by the inspection team. The team determined that TERA has

an adequate procedure for confirming the homogeneity of homogeneous design

activities and documenting the bases for review sample selection. However,

the team identified concerns in the procedure's implementation with respect to

scanning documents for homogeneity.

With respect to TERA's reviews, the team identified three areas where potential

" deviations"; i.e., failures to meet design criteria commitments or specifications,

were not found by TERA: component cooling water temperature calculations (0 pen

Items M-6 through M-9); compatibility of eductor flow rate with containment sump

pH requirements (0 pen Items M-12 through M-14); and potential adverse interactions

Items M-17 through M-19). In each of these areas, TERA needs to address the

specific concern and the generic implications, as well as to determine why the

TERA review process did not identify the items.

Items requiring IE followup are highlighted by open item numbers within Section 2

of the report. IE will address the open items and your responses during its

next DAP inspection, which will focus on the followup actions resulting from

TERA's identified discrepancies; e.g., safety significance evaluations and

corrective actions. It is noted that TERA has made numerous commitments as

a result of IE's October 28 to November 1, 1985 inspection which was documented

in inspection report 50-445/85-17, 50-446/85-14 dated January 21, 1986. These

are being tracked by a TERA commitment list (reference DAP-M-243 dated July 7,

1986), and will also require IE followup.

In accordance with 10 CFR 2.790(a), a copy of this letter and its enclosure

will be placed in the NRC's Public Document Room.
