We request amendment of USNRC Certificate of Compliance Number 9006 issued for our Type B(U) package Model AI 500 SU, USA /9006/B(U).

We ask that this amendment reflect a modification to the design of the source holddown arrangement.

This modification is designed to provide a greater degree of safety to the oper'ator during the loading and unloading of this package.

We are enclosing eight copies of the revised safety Analysis Report for this package.

We are also enclosing a check for $150.00 as the application Fee for this request.
