SUMMARY

Scope: This special, announced inspection of transportation activities resulted from the licensee's report of a UFgcylinder breaking loose from a trailer while in transit and falling onto a publft street. 2. Exit Interview The inspection scope and findings were summarized on July 10, 1986, with those persons indicated in Paragraph 1 above. The inspector described the areas inspected and discussed in detail the inspection findings. No dissenting comments were received from the licensee.

One violation concerning the failure to brace materials to prevent movement under conditions normally incident to transport was identified, based on the loss of control of one UF6cylinder on a shipment. The scope and findings of the review of the Confirmation of Action Letter were discussed on July 16, 1986. The licensee did not identify as proprietary any of the materials provided to or reviewed by the inspectors during this inspection.

3. Licensee Action on Previous Enforcement Matters This subject was not addressed in the inspection.

4. Description of Event
a. On July 7,1986, the licensee notified Region II by telephone that a UF6 cylinder being shipped from the licensee's facility to a Department of Energy facility had broken free from the truck and fallen on a street in the city of Lynchburg. The initial report indicated that no apparent damage to the cylinder integrity had occurred and no personal injuries had resulted.
b. Licensee representatives and local law enforcement agencies responded to the scene. Radiation and contamination surveys taken by the licensee indicated no release of the cylinder contents. No external contamination was found on the cylinder in excess of regulatory limits which further indicated that there had been no damage to the cylinder.

The cylinder vent valve cover and plugs were in tact and all seals were in place. Some superficial external damage to the cylinder was visibic, such as scraped paint, and dents in the cylinder neck ring.

c. The licensee arranged for a portable crane to position the cylinder back onto the trailer as well as to reposition the other cylinder still on the trailer. The cylinders were then chained to the trailer using the lifting pads on the cylinder. The trailer and the two cylinders were then returned to the licensee's facility and off-loaded for further inspection and storage.
5. Cause of Events
a. Each of two cylinders were placed in two supports (" saddles") on a flat bed trailer. Two restraints were placed over the cylinder and fastened to the connections on the trailer bed. The restraints for the cylinder which fell from the trailer consisted of two flat 4" fabric straps with hold downs which passed over the top of the cylinder but did not connect to the cylinder lifting lugs.

The support saddles were not attached or constrained to the trailer bed, except by the weight of the cylinder.

b. During transit, the truck made a rapid stop which caused the cylinder to move forward in the support saddles and put a twisting load on the straps. One strap failed in part due to dry rot of the material; the other strap failed by tearing. As the support saddles were not constrained they would not resist the forward motion of the cylinder and slid along with the cylinder. Upon striking the forward cylinder, the cylinder slid off the trailer, as did one support saddle. Ilhile it could not be determined which occurred first - the failure of the holddown strap or the sliding of the support saddle-both contributed to the event. Licensee representatives stated that they believed that the strap failed first, which allowed the support saddle to move.

6. Transportation Regulations
a. The UF c in the cylinders constitutes low specific activity radioactive materiM1. 10 CFR 71.5 requires that each licensee who delivers licensed material to a carrier for transport shall comply with the applicable requirements of the regulations appropriate to the mode of transport in 49 CFR Parts 170-189,
b. 49 CFR 173.425 specifies the requirements for transportation of low specific activity radioactive material. When shipped by exclusive use vehicle, the shipment must be loaded by the consignor, who in this case, was the licensee. 49 CFR 173.425(b)(6) requires that the shipment must be braced so as to prevent shifting of the load under conditions normally incident to transportation.
c. Since the load broke free and fell from the trailer during the stopping of the vehicle, a condition normally incident to transportation, the load was apparently not adequately braced. Failure of the licensee to comply with the bracing requirements of 49 CFR 173.425(b)(6) was identifieo as an apparent violation of 10 CFR 71.5. (70-1201/86-05-01)
7. Confirmation of Action Letter
a. Subsequent to the event, a Confirmation of Action Letter (CAL) was sent by NRC-Region II to the licensee on July 8, 1986. As a result of the CAL, the licensee discontinued all UF cylinder shipments until 6

completion of an NRC review

b. On July 10, 1986, the licensee sent a letter to Region II detailing the major elements of the corrective action plan to preclude recurrence during future shipments. The major elements included engineering analyses for the tie down and blocking methods, revision of the shipping procedure to address the tie down methods, inspection of the loading and tie down of cylinders, and training of personnel responsible for loading and inspecting the shipment,
c. On July 11, 1986, Region II sent a letter to the licensee stating that, after a review of the proposed corrective actions, there was no objection to the resumption of UF gshipments following implementation of the corrective actions. Shipm#nts of UF6 cylinders were resumed on July 11, 1986.
8. Inspection of Corrective Actions
a. On July 16, 1986, an inspector observed the preparation of a shipment of UFg cylinders and again reviewed the corrective actions taken by the licen5ee. The inspector reviewed Health-Safety Report No. S-001,
         " Engineering Evaluation of Tie-Down Configuration for Transport of UF 6 Cylinders", Rev. 14 to Procedure AS-1111, " Shipment and Receipt of Radioactive Material", and documentation obtained by the licensee 2

5 regarding-the design and construction of trailers used to transport the cylinders and of the test strength for hold-down straps.

b. The inspector observed the loading of two cylinders onto a trailer and the use of the inspection check-list of Procedure AS-1111. The inspector determined through discussion and review of records that the individual performing the inspections had received training on the requirements and was knowledgeable of the requirements. The inspector also determined that the trailer being loaded was one for which the licensee had documentation of the construction and was of the type considered in the evaluation.
c. The inspection results demonstrated that the licensee had satisfactorily completed the corrective action plan specified in the letter of July 10, 1986, which was the prerequisite for resuming shipments.
9. Enforcement Conference An Enforcement Conference was held at NRC Region II on August 21, 1986, to discuss the circumstances of the event, and the corrective actions taken by the licensee. The following persons were in attendance:
