Gentlemen:
This refers to the routine safety inspc-tion conducted by Messrs. D. S. Butler
and S. M. Hare of this office on May 25 through June 24, 1985, of activities
at Quad Cities Nuclear Power Station, Unit 2 authorized by NRC Operating
.
License No. DPR-30 and to the discussion of our findings with Messrs. R. Bax,
T. Tamlin and others of your staff at the conclusion of the inspection.
Additional information concerning this inspection was discussed on June 24,
1985 between the inspectors and Mr. T. Tamlin of your staff.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in non-
compliance with NRC requirements, as specified in the enclosed Appendix.
A written response is required.
As stated in the report the last two consecutive Type A tests at Quad-Cities
Unit 2 have been failurgs and, pursuant to 10 CFR 50, Appendix J, Section
III.A.6.(b), a Type A (.est shall be performed during each plant shutdown for
refueling until two consecutive Type A tests meet the acceptance criteria of
Appendix J.
Accordingly, unless you request, and are granted, an exemption
from this requirement by the Office of Nuclear Reactor Regulation, we expect
you to conduct a Type A test during each refueling outage until two consecutive
Type A tests satisfy the Appendix J acceptance criteria.
Unresolved item (265/85018-04(DRS)) identified concerns we have relative to
corrective action you have taken on excessively leaking containment isolation
valves. In your response to this inspection report you are requested to include
a list of containment isolation valves that have been consistently failing
their local leak rate tests, a summary of the corrective actions taken and your
current plans to preclude excessive isolation valve leakage in the future.

Throughout the course of the inspection, our inspectors noted evidence of
thorough preplanning and conscientious execution of the Type A test by Quad
Cities Station Personnel.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures will be placed in the NRC Public Document
Room.
The responses directed by this letter (and the accompanying Notice) are
not subject to the clearance procedures of the Office of Management and
Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
We will gladly discuss any questions you have concerning this inspection.
