NRC DOCKETS 50-321, 50-366 OPERATING LICENSES DPR-57, NPF-5 EDWIN I. HATCH NUCLEAR PLANT UNITS 1, 2 INSPECTION REPORT 86-36 Gentlemen:

The following information is submitted in response to Inspection Report Nos. 50-321/86-36 and 50-366/86-36, dated December 22, 1986.

The report concerns the inspection performed by the Senior Resident Inspector from October 18 through November 21, 1986.

NRC Notice of Violation:

" Unit-2 Technical Specification (TS) 3.5.1.b.2 [ sic] states "with both CSS subsystems inoperable, operation may continue provided that at least one LPCI subsystem is OPERABLE and both LPCI subsystems are OPERABLE within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />.

Otherwise, suspend all operations that have a potential for draining the reactor vessel and verify that at least one LPCI subsystem is OPERABLE within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br />."

Contrary to the above, between approximately 3:46 p.m. (EST) on November 13,1986, and 3:30 a.m. (EST) on November 14, 1986, with no activities in progress that had a pote itial for draining the reactor vessel, the operability of at least one Unit-2 LPCI subsystem was not verified within c

This is a Severity Level V violation (Supplement I)."

Response to NRC Notice of Violation:

Admission or Denial of alleged violation:

The violation occurred.

However, in accordance with 10 CFR 2 Appendix C.V. A., we request that I

The event was licensee identified.

It has been characterized as a Severity Level V violation, with no actual safety consequences.

It was reported as a Licensee Event Report.

It was corrected within four hours of discovery.

The corrective actions for previous violations would not have identified this violation because there has been no similar citation.

Accordingly, consistent with NRC's Enforcement Policy, the proposed violation should be withdrawn.

Reason for Violation:

The violation occurred due to a personnel error with respect to the review of the Integrated Leak Rate Test (ILRT) procedure, and subsequently reflected in an equipment clearance sheet, relative to the Technical Specifications requirements.

Corrective steps which were taken and the results achieved:

A more dstailed discussion of the corrective actions is contained in Licensee Event Report 50-366/86-35.

The ILRT procedure was immediately stopped and a thorough review of procedure 42SP-TET-003-25, " PRIMARY REACTOR CONTAINMENT INTEGRATED LEAKAGE RATE TEST PROCEDURE", was performed and the equipment clearance was reviewed.

The review was done, in a management critique, to determine if other discrepancies existed between the Technical Specifications and 42SP-TET-003-2S.

None were found.

Procedure 42SP-TET-003-2S was revised to correct the error.

A letter, on the proper method of clearance drafting and review, was written on December 11, 1986, and distributed to all licensed personnel.

In addition, the letter was reviewed with shift personnel.

As a result of the foregoing actions, compliance with the Technical Specifications should be assured.

This event and associated information have been provided to the Training Department for incorporation into initial and requalification training.

Corrective steps which will be taken to prevent recurrence:

No further corrective actions are necessary.

Date when full compliance will be achieved:

Full compliance has been achieved.
