Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(INSPECTION REPORT NOS. 50-327/86-62 AND 50-328/86-62)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

M. W. Branch on November 12-21, 1986. The inspection included a review of J

activities authorized for your Sequoyah facility. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

. representative records, interviews with personnel, and observations of activities

in progress.

The inspection findings indicate that certain activities appeared to violate NRC

requirements. The violation, references to pertinent requirements, and elements

to be included in your response are described in the enclosed Notice 'of

Violation. This violation involved a failure to translate vendor requirements

regarding Hydrogen Analyzer installation into the Sequoyah design.

This inspection report identified several items which Region II considers restart

items, they are: (1) operability of H2 analyzer paragraph 8.1, (2) evaluation

of Unreviewed Safety Question Determination (USQD) for Revision 3 -to the FSAR

regarding Hz analyzer accuracy paragraph 8.1, (3) radiation protection review

of penetrations in shield building paragraph 8.1, (4) followup of battery

room deficiencies paragraph 8.m, and the Engineering Change Notice closecut

and FSAR update items addressed in the December 18, 1986 NRC letter paragraph 5

and 9.

In accordance with Section 2.790 cf the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The responses directed by this letter and its enclosure are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
