GRAND GULF EXAMINATION REPORT 416/0L-86-03 The NRC administered requalification examinations on December 15-18, 1986, to employees of your company. At the conclusion of the writter, examination, a copy of the exam was left with your staff; at the conclusion of the site visit, generic findings on the operating examinations were discussed with those members of your staff identified in the Examination Report, Enclosure 1.

Copies of the written examination questions and answer key are included in this' report as Enclosure 2. Grand Gulf Facility coments regarding the written examination are included in this report as Enclosure 3. is the Requalification Program Evaluation Report, detailing the program evaluation.

The evaluation of the Grand Gulf requalification program was marginal based on the performance of the operators on the written examination. The NRC requests that Systems Energy Resources examine the written examination results to determine the cause of the marginal performance.

Systems Energy Resources should also determine what corrective actions are needed to enhance the knowledge of the failing operators and to correct any identified generic training deficiencies. Following this evaluation, we request that you inform us of your findings and planned schedules for future actions.

With regard to the individuals who did not pass the examination, it is our understanding that these individuals have been removed from licensed duties at Grand Gulf Nuclear Station and placed in an accelerated training program, as specified in your NRC1pproved requalification program. The NRC will review Grand Gulf's written reexamination, developed to confirm the success of its accelerated training program, for suitability prior to its administration.
