This refers to the routine team safety inspection conducted by Messrs.
~P. Byron, D. Kosloff, R._ Cooper, A. DeAgazio, J. Isom, M. Johnson, L. Kanter.
L.' Kelly, J. Smith, T. Stetka, L. Valenti, R. Westberg, L. Whitney, and
P. Wohld of this office from December 1,1986, through January 31, 1987, of
activities at Davis-Besse Nuclear Power Station authorized by NRC' Operating
License No. NPF-3 and to the discussions of our findings with Mr. L. Storz
at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice. A written response
is required.- Although the safety significance of each violation in the
-

Notice may be minimal, we are concerned that when viewed collectively, they
represent a decrease in attention to detail and awareness of the status of
plant equipment. Subsequent to the inspection, you requested a meeting to
discuss your recent operating experience. During the meeting held on
-February 27, 1987, you discussed a number of events and concerns, including
-those represented by the violations set'forth in the enclosure, and described
your corrective actions to minimize recurrence of these types of problems.
We will closely monitor the effectiveness.of your corrective actions. Addi-
tional violations of this type could lead tc escalated enforcement.
We are also concerned about an inadequate safety evaluation of a proposed
deviation from the Course of Action (C0A) commitment regarding the lineup of
the motor driven feedwater pump (MDFP) above 40% plant power (Paragraph 4 of
the inspection report). Your staff's verbal statement to NRR and to our
resident inspectors during the conference call on January 2,1987, indicated

that turning a switch in the control room was the only action required to
initiate flow to the main feedwater system and avoid an unnecessary Steam
and Feedwater Line Rupture Control System actuation. However, our resident
inspectors determined that a valve would also have to be manually closed
before starting the MDFP. This finding demonstrated that your review of
this matter was incomplete. Please-describe corrective actions you have
planned or taken to assure such reviews are complete.
In this regard, we
also are concerned by your slowness in responding to our resident inspectors
,
original request to formally document your conversation with NRR on these
. changes.
In accordance with 10 CFR 2.790 of the Consnission's reguiations, a copy of
this letter, the enclosures, and your response to this letter.will be placed
in the NRC Public Document Room.
