' DUKE POWER COMPANY RESPONSE TO VIOLATION 413/86-47-01 Violation 50-413/86-47-01
- Technical Specification 6.8.1 requires procedures to be established and implemenced for work on safety-related equipment. Technical Specification 6.8.2 requires each procedure of Specification 6.8.1 to be reviewed and approved by station management prior to implementation.
10 CFR 50.59(b) requires that a written safety evaluation be performed, which provides the bases.for the determination that a change, test or experiment does not involve an unreviewed safety question.
Contrary to the above, on October 24, 1986, while IB diesel generator was declared operable, low level vibrational testing was conducted on the IB diesel generator safety related load sequencer panel without the existence of a station management approved procedure and a safety evaluation had not been performed.
RESPONSE: -

Admission or denial of the violation:
Duke Power admits the violation.
(2)
Reason for the violation:
Personnel failed to provide a written safety evaluation and to have the vendor procedure approved by station management prior to the performance of an in-situ modal survey on the IB Diesel Generator sequencer panel.

The corrective steps which have been taken and the results achieved:
Work was temporarily suspended, while a work request and a safety a.
evaluation checklist were written.

The vendor-supplied procedure was reviewed and approved by station management in accordance with the Administrative Policy Manual (APM)
Section 2.7.1.3.2.

Diesel Generator 1B was declared inoperable until the survey was concluded.

The modal survey performed on the diesel generator load sequencer panel was part of a research project intended to investigate methods of evaluating the seismic impact of future modifications to similar panels.
The seismic qualification of this panel is documented by Wyle Laboratories Test Report.Jo. 45303-1, " Seismic Simulation Test Program j
on a Diesel Load Sequencer". It was determined at the conclusion of the survey, that actual vibratory amplitudes imposed on the panel were less than one-tenth of the levels to which the panel was qualified. The i
determination was thus made that the operability of the load sequencer l
was not affected.

Corrective steps which will be taken to avoid further violations:
Guidelines governing the use of vendor /off-site procedures performed on station safety-related equipment were reviewed with appropriate station personnel to emphasize when the need exists for reviewed and approved procedures. Also, Station Directive 4.2.1 has been revised to provide additional guidance in the use of vendor /off-site procedures at Catawba Nuclear Station. Such procedures will receive the same review and approval process as station procedures, including the performance of a safety evaluation.

Duke Power Company is now in full compliance.
