This refers to the special safety inspection conducted by Messrs. S. Hare,

D. Damon, and F. Clark of this office on November 10-21, 1986, of activities

at Quad Cities Nuclear Power Station, Units 1 and 2, authorized by NRC

Operating Licenses No. DPR-29 and No. DPR-30, and to the discussion of our

findings with N. Kalvianakis and R. Bax and others of your staff at the

conclusion of the inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

No violations of NRC requirements were identified during the course of this

inspection. However, significant deficiencies were identified in your

Licensed Operator Requalification Training Program. These deficiencies, as

documented in this report require that significant upgrades be made to your

requalification program. Subsequent to these program changes, the revised

Requalification Program Topical Report should be sent to the NRC for

approval. In the interim period, the effectiveness of your short and long

term corrective action will be monitored by Region III Operator Licensing

personnel through their periodic administration of requalification

examinations.

In accordance with 10 CFR 2.790 of the Connission's regulations, a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room.
