RESPONSE TO THE NOTICE OF VIOLATION REPORTED DURING THE HB.C INSPECTION CONDUCTED BETWEEN JULY 16 - AUGUST 19. 1988 INSPECTION REPORT NOS. 50-338/88-22 AND 50-339/88-22 NRC COMMENT During the Nuclear Regulatory Commission (NRC) inspection conducted on July 16 - August 19, 1988, one violation of NRC requirements was identified.

In accordance wich the "General Statement of Policy and Procedures for NRC Enforcement Actions," 10 CFR 2, Appendix C (1987), the violation is listed below:

A.

Technical Specification 6.8.1.a requires written procedures be established, implemented, and maintained covering access to the containment as documented in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Administrative Procedure (AP) 20.9, Containment Ingress and Egress, section 7,

has a note stating "Each department will complete the applicable sections of attachment I as the steps are completed".

Step 7.14 of AP 20.9 requires the health physics technician to determine the stay time using the sling psychrometer and document the wet and dry bulb temperatures and actual stay time on Attachment 1 of the proceduce.

Contrary to the above, the inspectors identifiEj that the licensee had not documented the wet bulb temperature or calculated the < tay time for the containment er.tries made on August 8, 1988 at 0929 and 1618.

This determination was made based on a review of Attachment 1 of AP 20.9 used fo', the containment entries, b.

Contrary to the above, during a containment entry made on August 9, the licensee failed to complete Attachment 1 of AP 20.9 as required by the procedure.

This is a Severity level V violation (Supplement I) and applied to Unit 1 only.

RESPONSE

1.

ADMISSION OR DENTAL OF THE ALLEGED VIOLATION The violation is correct as stated.

REASON FOR THE VIOLATION The violation was caused by personnel failing to implement the requirements of Station Administrative Procedure (ADM) 20.9.

CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED In an effort to ensure the importance of following station procedures is understood by the individuals involved in the event, the following corrective actions have been implemented:

A meeting was held with health physics supervision to review the event and delineate management's expectations for verbatim procedure compliance.

Health Physics shif t supervision discussed, with the health physics technicians involved in the event, how and when to obtain wet and dry bulb temperature readings.

Management's expectations concerning

,nrocedure compliance was also emphasized to the technicians.

In addition, disciplinary action was taken with appropriate personnel responsible for the work activity.

Recent performance during containment entries indicates that compliance with ADM 20.9 has been achieved.

There have been 29 containment entries since this event and no procedural noncompliances have been identified.

CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS No further corrective action is required.
