Inspection No. 50-247/86-28
This refers to the routine inspection conducted by Mr. Lawrence W. Rossbach
and Mr. Peter W. Kelley of this office on October 14, 1986 to November 21,
1986 at Indian Point Nuclear Generating Station, Unit 2, Buchanan, New York
of activities authorized by NRC License No. OPR-26, and to the discussions of
our findings held by Mr. L. Rossbach with you and your staff at the conclusion
of the inspection.
Areas examined during this inspection are described in the report enclosed
with this letter. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observations by the inspector.
Based on the results of this inspection, it appears that several of your
activities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation, enclosed herewith as Appendix A.
These vio-
lations have been categorized by severity level in accordance with the General
Statement of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2,
Appendix C (Enforcement Policy 1986). You are required to respond to this
letter and in preparing your response, you should follow the instructions in
Appendix A.
Of particular concern to us is the post-maintenance test issue identified
during this inspection. Testing performed after modifications failed to detect
that the capability of the auxiliary feedwater system had been significantly
degraded as a result of the modifications. While these particular items have
been corrected, your response should address actions taken to evaluate the
effect of this practice on other plant systems and components.
