ATTACHMENT TO G01-88-0060 DOCKET NO. 50-460 Potentially Reportable Condition 10CFR50.55(e)
Main Steam Isolation Valves Actuator Hydraulic 4-Way Valves
,Descriptioq of Deficiency The Supply System's WNP-1 Project utilizes four hydraulically actuated valves supplied by Anchor / Darling as the cein steam isolation valves.
Anchor /Carling rer.ently notified the Supply System that similar valves supplied to the Wolf Creek
- Flar, have encountered problems with slides cracking in the valve hydraulic actu.or 4-way valves.
This problem has been reported to the NRC by both Wolf Creek and Anchor / Darling.
Ana1ysis of Safety Isolication Failure of,io hydraulic actuator 4-way va'.ve coul/ ufect the ability of the main steam isolation valves to close and provide the required isolation.
Cause of Deficiency Af ter evaluating the problem in depth, Anchor / Darling has concluded that the cracks resalt frcm brazing / heat treating of the slide material.
The WNP-1 valves have not been operated and the Supply System has not performed an independent evaluation of the condition.
Corrective Action Anchor / Darling has provided test procedures to identify 4-way valves with ootentially cracked slides. During construction restart, the 4-way valves will be tested and the necessary repair or component replacement completed prior to placing the main steam isolation valves in service.
Action to Prevent Recurrence No further action is deemed necessary following the 4-way valve testing and completion of any necessary repair or component replacement.
