FEMA REGION VI FINAL REPORT FOR REMEDIAL DRILLS CONDUCTED AT THE GRAND GULF NUCLEAR STATION Enclosed are copies of correspondence received from FEMA documenting their evaluation of the remedial drills which were conducted on May 14, 1986, and August 6-7, 1986.

According to FEMA correspondence dated November 24, 1986 (see memo from Richard W. Krimm to Edward L. Jordan), the deficiencies have been either corrected or scheduled for correction.

Based on discussions with Mr. L. Dale of Mississippi Power and Light, we understand that problems involving the dedicated hot line telephone system will soon be resolved or are in the process of being resolved.

We encourage your continued assistance to the States and counties in the development of a scenario for the next full scale exercise that will test as many of the areas as practical in which the previously identified deficiencies were found.

We outstanding deficiencies that have been corrected or are scheduled for correction ares o limitations of the primary dedicated hot line telephone system in the State Dnergency Operations -Center; This deficiency is expected to be corrected by December 31, 1986.

o unsatisfactory direction and control of the field monitoring teams including connunication problems involving the Louisiana Nuclear Energy Division; This deficiency was corrected by the installation of a new two-way radio base station at the EOF.

o unsatisfactory handling of contaminated individuals by the Madison Parish Hospital and Anbulance Services; This deficiency es corrected by the identification of a different hospital to provide radiological handling of contaminated individuals and the successful remedial drill of the Mahoney Ambulance Service.

The appropriate state plans have been anended to reflect this change.

. Based on the results of the exercise, remedial drills, and scheduled corrective actions, the of fsite radiological energency plans and prepared-ness are adequate to provide reasonable assurance that appropriate masures can be taken to protect the health arxl safety of the public in the event of an accident at the Grand Gulf Nuclear Station; and, the 44 CFR 350 approval granted in June 1983 will renain in effect.

SUMMARY

J 1.1 REMEDIAL DRILL BACEGROUND Following the December 3-4, 1985 full participation exercise, a remedial drill m

I was required. This drill was conducted May 14,1986 to correct deficiencies identified in

the December,1985 exercise. Evaluation results of the May 1986 remedial drill required q

a second remedial drill in the areas of: 1) field monitoring activities, and 2) medical

~'j support (ambulance only).

Therefore, a second remedial drill was conducted and I

evaluated on August 6 and 7,1986.

This report includes evaluation of the second l

y remedial drill.

i Three deficiencies were identified as a result of the December 3-4, 1985 exercise.

These deficiencies were documented in the March 4, 1986 report.

description of the defic!encies and their current status follow.

w Primary Dedicated Hotline System: As identified in the March 4,

,_l 1986 report under deficiency #1, this system did not allow the State of Louisiana opportunity to initiate conversations with affacted parties. FEMA has reviewed a letter from Mississippi Power and Light to the State of Louisiana informing them that the operational

~~J hotline (OHL) upgrade has been ordered from AT&T. This system upgrade will give each party the ability to selectively ring any other l

party. The "ringback" system is expected to be in service no later than 12/31/86. Therefore, the defielency has been corrected and FEMA will verify the system's operability.

Madison Parish Hospital and Ambulance Service: A deficiency (#20-March 4,1986 Report) and two areas requiring corrective action (#'s

,]

21 and 22 March 4,1986 Report) were identified as a result of the December 3-4, 1985 exercise, An exemption request regarding

~'

Madison Parish Hospital and two others, of their REP support

_l responsibilities was approved on August 28, 1986 by FEMA. Thus,

, J objective 31 is no longer applicable to Louisiana hospitals and, therefore, the deficiency and ARCAs relating to hospital REP responsibilities in Louisiana have been eliminated.

However, medical support with regard to ambulance service responsibilities still exist and these capabilities were tested during the second remedial drill by the Mahoney Ambulance Service and the Louisiana Nuclear Energy Division.

The results of the demonstration of FEMA objectives #30, #5, and #36 in response to the deficiency and ARCAs are include'd in this exercise report in Section 2.3.

l Direction and Control of Field Monitoring Teams: As indicated in the March 4,1986 exercise report, the direction and control of field monitoring teams was unsatisfactory during the December 3-4,1985 J

exercise. The conveyance of information, from the LNED staff at l

the EOF, to the field monitoring teams via the field team j

coordinator and dispatcher located at the Tensas Parish EOC, was awkward and inefficient. This arrangement resulted in the field teams not being effectively tracked, and inaccurate information J,

being transferred between the LNED staff at the EOF, the field team coordinator and dispatcher at Tensas Parish, and the field monitoring teams. As a result of these evaluations, deficiencies were identified for FEMA objectives #3, #4, and #36 and the first remedial drill was req'uired.

In response to FEMA recommendation, a new two-way radio base station was installed at the EOF to allow LNED personnel to control field monitoring teams directly from the EOF, thus eliminating the need for cumbersome communication link through Tensas Parish.

The Tensas Parish Courthouse complex continued to be used as' a i

staging area for the field monitoring teams.

During the first remedial drill, direction of field monitoring teams from the EOF proved to be very effective. This demonstration corrected deficiencies #11 and #12 documented in the March 4, 1986 exercise report.

Most Areas Requiring Corrective Action related to field monitoring were also corrected during the first remedial drill.

However, one of the two State field monitoring teams (Team #2) participating in the first remedial drill, failed to adequately demonstrate FEMA Objectives #7 and #8. Therefore, as previously stated, a second remedial drill was required to re-demonstrate FEMA Objections #7 and #8. The second remedial drill was conducted on August 7,1986. Evaluation results in Section 2.2 of this report describe how these two objectives were adequately demonstrated.

1.3 REMEDIAL DRILL OBJECTIVES

The State of Loulslana agreed to retest the following FEMA Objectives (listed by FEMA Objective No.) to correct the associated Deficiencies and/or Areas Requiring Corrective Action.

1st Remedial Drill conducted May 14,1986 EOF Objectives FEMA #3 Demonstrate ability to make decisions and to coordinate emergency activities (Deficiency #11 and #12 - 3/4/86 report).

FEMA #4 Demonstr adequacy.of facilities, equipment, maps and displays to support gency operations.

FEMA #5 Demonstra ability to comraunicate with all appropriate locations, anizations and field personnel (equipment)

4 Field Monitoring Teams Objectives In addition to the objectives listed above, the following two objectives were tested by the field monitoring teams:

6 2 EXERCISE EVALUATION The Emergency Operating Facility and two field monitoring teams participated in the first remedial drill conducted 5tay 14,1986 to demonstrate objectives to correct the identified Defielencies and Areas Requiring Corrective Action based on the December 3-4, 1985 exercise. The following includes a narrative summary of the EOF performances in the afay 14, 1986 drill. The narrative summary for the two field teams is the performance evaluation noted in the sec',nd remedial drill conducted August 7, y

1986. The evaluation is included in section 2.2 of this report.

The numbering of defielencies and ARCA's in the narrative sections that follow correlate with the Afarch 4,1986 report that documented the results of the 12/3-4/85 exercise.

However, the numbering of deficiencies and ARCA's in Tables 1-3 of this report reflect their addition to the list of deficiencies and ARCA's identified at the 12/3-4/85 exercise, therefore Tables 1-3 are a combined list of deficiencies and ARCA's identified at the 12/3-4/85 exercise and the first S/14/86 remedial drill. There were no deficiencies or ARCA's in the second August 6-7, 1986 remedial drill. Credit was given for previous deficiencies and ARCA's which were corrected during these drills.

2.1 EOF FIELD TEAM COORDINATION NARRATIVE The EOF was activated only to test the direction and control of field monitoring from this location through the use of the new two-way radio system. The LNED Staff consisted of the field team dispatcher and an administrative assistant.

The field teams were staged at the LNED trailer at the Tensas Parish Courthouse complex. Prior to deployment, both teams were briefed by the LNED Field Stonitoring Team (F31T) coordinator at the EOF based on the initial conditions presented. Field team direction and control during the first and neond remedial drills were greatly improved with the direction from the EOF. The field teams were effectively positioned, tracked and directed to monitor the edge of the plume.

Decision-making sad coordination of the field teams from the EOF by the LNED F31T coordinator demonstrated correction of Deficiencies 411 and 412 as documented as a result of the December 1985 exercise and presented in the Starch 4,1986 report.

Appropriate maps and visual aids were available to track the location of the field monitoring teams. To improve the plotting of previous locations of the field monitoring teams, it was suggested after the 1st remedial drill that adhesive paper be used which provides space for type of sample, reading, time of sample, and any other pertinent information to track the teams. This area recommended for improvement was carried out at the second remedial drill.

Installation of a new radio at the EOF greatly improved the ability to coordinate the field monitoring teams, as comparec. with the December 3-4, 198a exeretse.

I Communications with Field Team #2 was intermittent for a period of approximately five minutes but was quickly corrected. This problem was likely due to an intermittent connection in the new van, which had a radio installed in it the day before the exercise.

teams would contact the FMT coordinator via commercial telephone. Coins for use at a pay telephone were provided to each field monitoring team.

Although communications from the EOF were excellent, minor changes would provide a more workable system. Earphones should be available for the EOF radio to l

facilitate effective communications with the high ambient noise levels such as could be

expected inside the EOF during an actual emergency. The FMT coordinator indicated that another radio " control head," equipped with a combination headphone and boom microphone, is being readied for use at the EOF. Improved radio protocol from the FMT coordinator could help to eliminate delays in the acknowledgment from the field monitoring teams and the simultaneous transmission (" doubling") by the field monitoring teams af ter a FMT coordinator announcement. One possible improvement would be for the FMT coordinator to poll each field monitoring team in sequence after each announcement.

Also, closer attention to proper station identification would help to eliminate confusion in the event of an actual emergency. Although most of the radio protocol Issues were identified during the December 1985 exercise and the 1st remedial drill, improvements were made during the second remedial drill.

Based on these evaluations for communications procedures and equipment, the Deficiencies (#11,12,15) identified in the March 4,1986 report, were corrected.

The field team coordinator / dispatcher was also assisted by a secretary who logged exposures of emergency workers. Dosimeter readings were collected every 30 minutes and exposures were logged at the EOF. Based on this demonstration, FEMA Objective #20 was met.

Overall, all FEMA objectives were met at the EOF and the previously identified deficiencies were satisfactorily addressed.

2.2 FIELD MONITORING TEAMS NARRATIVE Two field monitoring teams were activated for the first remedial drill on May 14, 1986. Field Team #1 successfully demonstrated Objectives 5,7,8,20,36, and corrected that Team's deficiencies and Areas Requiring Corrective Action.

However, they only partially demonstrated Objective #20, and deficiencies in their performance of FEMA Objectives

FIELD 'IEAM #1 Team #1 made proper and professional measurements of both open and closed readings of ambient radiation, following current procedures.

Ground level measurements were not taken but are not required by the current procedures.

Air samples were taken and measured following current and proper procedures. The air sample was taken using a jig on the front of the van to hold the air sampler at the proper height above the ground. The cartridge and filter holder were covered with a single layer of plastic bag while being carried into the van. This prevented contamination of the vehicle or field team members during bagging and measuring the filter and cartridge inside the vehicle.

A double plastic bag would present an extra barrier of safety in handling the cartridges in the vehicle. Samples were measured after the team moved to an area of background ambient radiation. Samples were properly double bagged and labeled. The cartridges were measured properly using a specially designed jig to assure proper geometry. The filter paper and cartridge

9 were both checked to see if they could be safely stored in the van without excess radiation exposure to those in the van.

10 FIELD TEAM #2 The field monitoring teams were mobilized from their work locations in Baton Rouge. This procedure would be follovmd during normal duty-hours.

The field teams have lockers with supplies in place and a checklist is used to inventory equipment. Team #2 inventoried its locker prior to departing from Baton Rouge and found it complete. The field team was comprised of two members, the lead health physicist and an assistant health physicist. Members were also given briefings on plant conditions, weather, etc.

In the field, good procedures were followed in taking an air sample.

The sample was double-bagged and was appropriately labeled. Team #2 is also commended on its use of the Ludlum 14C with 1"x 1" scintillator for early and very sensitive detection of gamma radiation.

There was a communications check between survey teams early on during deployment and it caused Team #2 to note that its transmitter was not operational. The Team 'immediately turned back and traded vehicles (for one with an operational radio).

Field Team #2 practiced good exposure control; all protective equipment was available, KI was on hand; and all appropriate dosimetry equipment was available. Dosimeter readings were made every 30 minutes and reported to the Field Monitoring Team coordinator at the EOF. Team #2 knew dose limits. Team #2 adequately demonstrated FEMA objectives #7 and #8 during this second remedial drill.

y 11 2.3 MAHONEY AMBULANCE SERVICE (EMS)

NARRATIVE Mahoney Ambulance Service is the designated emergency medical service for transporting injured, contaminated persons during a nuclear emergency in the St. Joseph, LA/Tensas Parish area. Training of EMS personnel in the proper handling of an injured, contaminated individual was conducted by the Louisiana Nuclear Energy Division and Mahoney Ambulance Service.

FEMA objective #30 was adequately demonstrated by Mahoney Ambulance Service.

Ambulances used by the firm are equipped with two sets of anticontamination clothing, two 0-200R dosimeters and a CDV-700 radiation survey meter. The ambulances are also equipped with a two-way radio system capable of communications on the Sheriff's Network, the Louisiana and Mississippi Hospital Network, the Civil Defense Network and a phone patch communication system.

At 1:30 p.m. a telephone call was received at the Tensas Parish Sheriff's office informing them that someone had been shot with a rifle (simulated) at the junction of Highway 604 and 605. According to the caller, the injured person and his companion had heard the siren but were unable to leave the area or take shelter. Mahoney Ambulance Service was contacted at 1:30 p.m. using the Civil Defense Network and was dispatched to the scene.

The ambulance service's ability to use emergency communication equipment to communicate with other organizations was demonstrated satisfactorily by this portion of the exercise. Thus, FEMA objectives 5 and 36 were met.

Due to the expense and the time necessary for replacement, complete dress out by the EMS crew in anticontamination clothing was simulated for this exercise. At 1:35 p.m. the EMS crew arrived at the junction of highway 604 and 605 and was met by the wounded (simulated) person's companion. In order to assess the extent of the injuries, it was necessary to remove the wounded individual's shirt. To prevent the spread of any contamination in the ambulance, the EMS crew bagged the victim's shirt and wrapped him in a blanket prior to loading him for transport. The wounded person's companion was transported in the ambulance and was instructed to wrap herself in a blanket.

In order to demonstrate off loading of the patient and decontamination of the ambulance, a hospital emergency room entrance was simulated at the Tensas Parish Civil Defense office.

In an actual emergency, the wounded individual would have been transported to the nearest hospital (Tensas Parish Hospital for patient stabilization) and T

then transferred to the Vicksburg or Port Gibson, MS Hospital for radiation management.

At the simulated hospital, a hotline and radiation control area was established by members of a field monitoring team from the Louisiana Nuclear Energy Division (LNED).

A radiation survey of the transport vehicle, EMS crew and passengers was conducted. The patient's companion was determined to be contaminated and appropriate decontamination procedures were instituted. All contaminated clothing, blankets, etc.

were collected by the field team and double bagged for proper disposal. The LNED

12 Team did an excellent job in monitoring the vehicle, equipment, patients and ambulance crew. They were knowledgeable about proper decontamination procedures.

The adequacy of EMS transportation, personnel and procedures for handling contaminated individuals, including decontamination of vehicle and equipment was demonstrated satisfactorily during this portion of the exercise. Thus, FEMA objective 30 was met. This demonstration also provided corrective action for ARCA #21 from the December 3-4, 1985 exercise.

31 4 EVALUATION OF OBJECTIVES 4.1

SUMMARY

OF FEMA OBJECTIVES REMAINING TO BE MET Table 2 on the follcwing pages provides listing of those FEMA objectives which, according to the FEMA RAC Chairman, have not been satisfactorily met or tested and which should be incorporated into the exercise objectives on or by the sixth year of the six-year period in which all the objectives must be tested. These should be considered in j

the development of future exercise objectives; as well as those FEMA objectives which,

,r although previously tested and satisfactorily demonstrated, must be tested and evaluated during any full-participation exercise of offsite state and local response capabilities.

4.2 FEMA OBJECTIVES TRACKING - GRAND GULF STATION Table 3 provides comprehensive tracking system of FEMA Objectives, NUREG-t 0654 Reference Elements, Exercise Objectives, Jurisdictional Responsibility, Exercise Dates, Identified Deficiencies and Required Corrective Actions, and the Date Specific FEMA Objectives Were Met by state and local agencies. This system will track the progress and status of this data through the six-year exercise cycle in which all FEMA objectives must be tested.

NOTE: FEMA Objective No. 31 is shown as not applicable to Tensas Parish regarding the Grand Gulf Station offsite plans. This is based on a special exemption request (Hospitals only) submitted by the State of Loulslana and approved by FEMA on August 28, 1986. Loulslana offsite REP plans have been amended to clarify that any injured, contaminated patient would be transported to one of the two desig-nated contract hospitals in Mississippi. If a patient needed immediate life saving emergency care and stabilization, the patient would be first transported and treated at the nearest Louisiana hospital and then transferred for radiological treatment and care at one of the two Mississippi hospitals as per the amended plans.
