NOTICE OF VIOLATION Sacramento Municipal Utility District Docket No. 50 312 P. O. ;ox 15830 License No. DPR-54 Sacramento, California 95813 During an NRC inspection conducted on July 9 through September 6, 1988 violations of NRC requirements were identified.
In acccrdance with the i
"General Stctement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Par t 2, Appendix C, the violation is listed below:

Regulatory Guide 1.33, November 1972, states, in part tt.at safety related activities which should be covered by written procedures (include):
"A.4.
Procedure Adherence and Temporary Change Method,"
Rancho Seco Administrative Procedure AP-26A, "Temporary Hodification Control", Paragraph 4.2 states, in part:
"This procedure shall be used for safety-related and non-safety-related systems and provides requiraments for controlling and documenting temporary modificJtions..."
Paragraph 4.3.2 states, in part:
"...if the temporary alteration must remain installed when the equipment is returned to service, a temporary modification is required (per this procedure) to be approved and in place prior to j
returning the equipment to service."
Contrary to the above On August 6, 1988, the temporary modification of the outboard seal of auxiliary feedwater pump P-318 was not controlled and documented per AP-26A.
The seal was modified to install only 5 of 6 specified packing rings during troubleshooting of overheating of the packing.
The equipment was returned to service and declared operable with the modified seal installed.

Document Control Desk, Washington, DC 20555 with a copy to the NRC Resident Inspector, Anthony D'Angelo, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a "Reply to a Notice ot' Violation" and should include for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achioved.
If an adequate reply is not received within the time specified in this Notice, an order may be issued tc show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
Consideration may be given to extending the response time for good
- ause shown.
