SUBJECT: LASALLE INSPECTION REPORT 50-373/99001(DRP); S0-374/99001(DRP)

Dear Mr. Kingsley: ,

This refers to the inspection conducted January 6 through February 16,1999, at the LaSalle j

Nuclear Power Station. The purpose of the inspection was to determine whether activities I

authorized by the license were conducted safely and in accordance with NRC requirements. At

the conclusion of the inspection, the findings were discussed with those members of your staff

identified in the enclosed report. No cited violations of NRC requirements were identified during

the course of this inspection, j

Overall, performance of plant personnel was acceptable. Operators were knowledgeable of

plant and equipment status, maintained accurate records, and effectively communicated  !

operational information. In two instances, which included a loss of the heater drain pump

supply to the reactor feedwater pumps and a decrease in the main condenser vacuum while

realigning the off-gas system, operators appropriately responded to the unplanned plant

transients which resulted from equipment prcolems. The decrease in condenser vacuum was

attributed to the leaking valves in the off-gas system and although the plant responded as

designed, plant material condition impacted operations.

Personnel errors occurred during the inspection period. Inattention to detail and lack of a l

questioning attitude by maintenance and operations personnel resulted in lapses in

configuration control and some problems with the foreign material exclusion (FME) program,

For exarnple, operators inadvertently drained 25,000 gallons of condensate to the Unit 2

suppression pool through a drain valve that was not closed, construction personnel terminated

electrical leads onto a breaker while out-of-service cards were affixed to the leads, and

maintenance personnel did not adequately secure an FME cover onto a disassembled

emergency core cooling system valve. Although none of the issues was safety significant, they

demonstrated the continuing need for management attention to address human performance at

LaSalle Station.
