SUBJECT:

MAINTENANCE SURVEY AT RANCHO SEC0 NUCLEAR GENERATING STATION We plan to conduct a survey of the maintenance and surveillance program at Rancho Seco as part of our restart evaluation. We will schedule the survey after you conclude that the station maintenance program improvements are completed. We suggest that when you conclude that your improvement program is essentially complete, a meeting be held with us at Bethesda, Maryland to review the status of the program and to establish a date for our survey tem to visit Rancho Seco. We are currently targeting the survey during early February 1987 in order that the safety evaluation input on this issue does not become the pacing item in our restart safety evaluation. Please contact our Project Manaaer, Sydney Miner, to set a date for the meeting.

To assist in scheduling the survey, we are providing in (a) a description of the core activities to be performed by the survey team (Enclosure I) and (b) a list of documents needed by the NRC prior to the above proposed (Enclosure II) meeting.

The reporting and/or recordkeeping requirements contained in this letter affect fewer than ten respondents; therefore, OMB clearance is not required under P.L.96-511.

Enclosure I

Subject:

Rancho Seco Site Survey

Purpose:

To collect descriptive data and observations related to the Maintenance and Surveillance Program. The following areas will be reviewed:

1) Corporate Goals Concerning Maintenance
2) Spare Parts / Material Readiness
3) Supervision Methods in the Maintenance Department
4) Preventive / Predictive Maintenance Practices
5) Maintenance Work Order Backlog
6) Maintenance Procedures -
7) Maintenance Work Control 3 8) Communications
9) Definition of Responsibilities in the Maintenance Organization

11) Maintenance Work Activity Planning and Scheduling
12) Administrative Procedures Survey Method: Using an interview technique in informal questions and answers session with plant personnel ranging from plant management to foremen ,

and craft personnel. The duration of an interview is expected to be 20 to 30 '

minutes.

Interviews will be conducted by a team of five NRC staff. This team will be divided in two groups (sub-teams) and will be on site for 1 week duration.

  • Provide five Completed Maintenance Work Order Packages (randomly selected).

Description of shift work and work assignment (e.g., how foreman decides on which craft to perform what type of work).

Complete description of the preventive maintenance program:

- How equipment is to be included

- How frequency is determined

- How execution is planned

- How it is going to be updated ,

Complete description of predictive maintenance program:

- Which equipment is included

- What methods used: oil analy. , vibr. , thermography, etc.

- Methods of record keeping

- Is it going to be updated Concerning management:

- Are there goals set for the maint, dept.

- What schedule?

- What milestones?

- Are goals in performance evaluation of managers and supervisors?

- Are goals communicated to first line supervisors and crafts?

Maintenance Work Orders (MWO)

- Current number of non-outage corrective MWO by priority

- Definition of priorities

- Projected number of corrective MW0s to be outstanding at start-up by priority

- Current number of preventive maintenance work orders over due

- Rate of completion of corrective MWO in terms of number completed / month and manhours expended (by craft)/ month for the past 4 months

- Estimated manhours required to complete MW0s both current and projected at start-up broken down by craft

- Rate of completion of preventive MWO in terms of number completed / month and manhours expended on PM MWO/ month for the past 4 months

- Estimated manhours required to complete current overdue PM MW0s

- Status of current MW0s (e.g., number in planning, number in final sign-off, number on hold for lack of parts, number on hold for engineering assistance, number available to be worked on)

- Number MW0s requiring rework over past 6 months.

Enclosure II To help in the preparation of the site survey, we ask you to provide us with the following documents:

  • Administrative Procedure: Maintenance and Predictive and Preventive Maintenance activities. ,

Organization Chart (with names) for maintenance organization and plant-wide organization.

Composition of craft personnel for: Electrical, I&C, Mechanical (Foreman to craft ratio, etc.). -

  • Description of Planning Department / Activity.

Description of daily meetings for maintenance planning, scheduling, status reports, etc.

Description of Maintenance / Operation Interface during planning, scheduling, work close-out, and post-maintenance / functional testing.

Description of Work Control Process: i.e., how work order is started, planned, executed, completed, closed out and equipment is returned to

service. Provide procedure.

Description of craft training and retraining requirements.

Average years of experience for each craft discipline. Turnover rate by craft Description of interface /communir.ation between Technical / Engineering support staff / Plant Modificationa/QA-QC/ Maintenance Department

  • Description of procedure control: initial write-up, validation, revision or upgrade. Technical review and human factors review criteria.

Requirements for conduct of maintenance work, troubleshooting, work closed out, post-maintenance / functional testing, and returning of equipment / system to normal line-up.

Description of methods in which the performance of the maintenance department is measured, e.g., are indicators used, how frequently, who is informed?

Description of process on communication with vendor for technical services and latest technical information on equipment and systems installed at plant site. Interface with vendor /NSSS for training, modification and replacement.
