Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/88-28 AND 50-328/88-28)

This refers to the special Nuclear Regulatory Commission (NRC) thift coverage

inspection conducted by the NRC's Sequoyah Restart Task Force on May 5 - June 2,

1988. The inspection included a review of the activities associated with the

startu) of Secuoyah Unit 2. At the conclusion of the inspection, the findings

were ciscussec with those members of your staff identified in the enclosed

inspection report.

Areas examined during the inspection are identified in the report. These areas

included extended control room observation and operational safety verification.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

observation of activities in progresti.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The identified violation involved the failure to follow

procedures. Accordingly, the violation references to pertinent requirements,

and elements to be included in your res,ponse are described in the enclosed

Notice of Violation. In addition, inspection findings have identified what

appear to be additional exam)1es of a previous violation (327,328/88-26-01)

concerning configuration con;rol. The additional examples are addressed in.

paragraph 9 and 10.d of the enclosed inspection report. Please provide a

response within 30 days of the date of this letter, which will address whether

the corrective actions implemented to correct violation 327,328/88-26-01

adequately encompass these additional examples.

In accordance with Section 2.790 of the NRC's "Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a cosy of this letter, its enclosures,

and any reply will be placed <n the NRC's Pu)1ic Document Rcom.

The responses directed by this letter and its enclosures are not subjected to

the clearance procedures of the Office of Management and Budget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
