DUKE POWER COMPANY McGUIRE NUCLEAR STATION RESPONSE TO VIOLATION IN INSPECTION REPORT 50-369/86-26 AND 50-370/86-26 Violation 50-369/86-26-01 and 50-370/86-26-01, Severity Level -IV (Supplement I):
10 CFR 50, Appendix B, Criterion XVI, as implemented by Topical Report Duke-2-A, Section 17.2.16, requires measures which assure that conditions adverse to quality are promptly identified and corrected.
For significant conditions adverse to quality, Criterion XVI requires that the measures assure that corrective action is taken to preclude repetition.
Contrary to.the above, measures taken by the licensee to assure that repetition of a previously identified. Violation was precluded were inadequate. NRC Violation 369, 370/86-05-01, cited the licensee for their failure to perform required verifications of proper functioning of remote position indicators for safety
' injection system sump suction valves. The licensee's corrections to provide for performance of the verifications were inadequate, in that:

Procedures PT/1/A/4200/22 (Unit 1) and PT/2/A/4200/22 (Unit 2) were modified to include the required verifications. However, successive procedural steps-intended to verify correct indication for suction valve NI-184B (both Units) first specified closure of the valve and then specified-(incorrectly) verification that the position indicator showed it to be open.

Records of.the laat performance of the above procedure revealed that personnel who performed the procedure had first closed valve NI-184B (as indicated by their initials) and then verified that the position indicator showed it open (indicated by initials).
Response

Admission or denial of the alleged violation:
Duke Power admits the violation occurred as stated.

Reason for violation:
The violation resulted from an oversight with the reissue of the entire procedures. Also, the individuals conducting the tests were not reading the steps fully while performing the action.

Corrective steps which have been taken and the results achieved:
The immediate response was to question the individuals who conducted the tests to ensure that they actually did observe the valves going closed.
Procedure changes were initiated to correct the completed procedures and the control copies of the procedures. These were implemented by the individuals who conducted the tests.

Corrective steps which will be taken to avoid further violations:
s-4 The individuals involved were counseled to read each procedure step in its entirety to ensure full compliance with station and NRC regulations.
This violation will be covered with the entire Performance section and documented as required training.

3, The procedure changes have been implemented. Training will be completed by January 31, 1987.
