Inspection 50-289/86-06
During the period from April 11, 1986, to May 16, 1986 members of the NRC
Region I Staff conducted routine safety inspection activities at your facility
with respect to cold shutdown outage and return to power operations. The in-
spectors documented the results of these inspections in the enclosed inspection
report. At the conclusion of the inspection, Mr. F. Young, of my staff, sum-
marized the inspection findings for you and other members of your staff.
As a result of our inspection, we identified one apparent violation of NRC
requirements to properly adhere to procedure (Appendix A) and an apparent
deviation of your commitments to us in the area of radwaste supervisory
training (Appendix B). You should follow the instructions in the Appendices.
The violation addresses several examples of not properly implementing impor-
. e.,
' tant-to-safety procedures.
Based on this and other examples of weak procedure
implementation noted in the inspection report, it appears that your procedure
adherence problem persists. We plan to discuss this issue with you in the
upcoming SALP management meeting.
Also, our followup of an environmental qualification issue from the NRC Per-
formance Appraisal Team inspection is detailed in paragraph 10 of the enclosed
report.
It identifies three examples of a failure to meet EQ requirements.
This matter requires additional NRC review; thus, our enforcement action will
be forwarded later under separate correspondence.
