On January 11,1999, the NRC completed an inspection at Millstone Units 2 & 3 reactor

facilities. Millstone Unit 1 was also inspected as part of the site-wide review of the program for

the processing of radioactive waste and transportation of radioactive materials.

During the 7-week period covered by this inspection report, your conduct of activities at the

Millstone facilities was generally characterized by safety-conscious operations, sound

engineering and maintenance practices, and careful radiological work controls.

Although three items on the Unit 2 Significant items List (SIL) were inspected and closed,

corrective action for one SIL item was found to be incomplete at the time of the inspection. Your

completion of corrective actions subsequently allowed for NRC review and closure of the item.

Wdh 34 SIL items still open, corrective actions described in the closure packages should be

complete and timely.

At Unit 2, in response to the NRC's request for information pursuant to 10 CFR 50.54(f), on

- December 1,1998, you provided the latest update to the Unit 2 deferred items list. Our review

of the deferred items found that your decision-making process for the identification of items to be

,

completed after the restart of Unit 2 was conservative and thorough. The process resulted in

the list containing items that were appropriate for deferral. The NRC did not identify any issues

- that, if not corrected prior to plant restart, could have resulted in a significant safety concern

during plant operations. Unit 2 SIL No.12 remains open pending NRC review of an additional

. update to the deferred items list that you are required to submit prior to restart.

During this inspection period, the Unit 3 staff responded adequately to an automatic reactor trip,

subsequently taking the plant to cold shutdown conditions to effect design modifications to the

main steam isolation valves. Your plant staff also completed event review and corrective

measures in response to a tank leak that resulted in the unplanned release of a small amount of

radioactive material through an unauthorized, unmonitored path to the environment. Each of

. these events illustrate the need for certain program changes and improvements. These events

are similar to the other reactor trips and plant transients that have occurred since the unit startup

in July 1998.

The number and nature of these continuing plant and operational challenges, coupled with the

demands placed upon your staff resources for timely response and backlog reduction, dictate

the need for an efficient work control process and an appropriate prioritization of the workload.

Continued management attention to improve Unit 3 operational focus is appropriate.
