Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/88-27AND50-328/88-27)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted

by K. Jenison on May 1,1988 thru May 30, 1988. At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within

these' areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The inspection findings indicate that certain activities appeared to violate

NRC requirements. The identified violation involves the failure of your

corrective action programs to assure that violations of your Technical

Specifications with regard to making positive reactivity changes in conjunction

with an inoperable Control Room Emergency Ventilation System will not recur.

The violation, references to pertinent requirements, and elements to be

included in your response are described in the enclosed Notice of Violation.

Although the specific technical aspects of this violation are not significant

safety concerns, the following programmatic issues do concern us.

1. The Condition Adverse to Quality Report (CAQR) related to these

events specified inadequate corrective action, was incorrectly closed

based on a temporary procedure change and did not require a root

cause analysis or recurrence control.

2. The Plant Operations Review Staff was apparently not knowledgeable of

the corrective action status or actual actions accomplished at the

time Revision 1 to LER 327/870078 was prepared and issued.

3. Your Corporate Commitment Tracking System closure procedure does

not appear to contain adequate controls to assure that proper and

complete actions have been taken prior to closure of the tracking

item.
