SpecialInspection No.93-014 On September 30,_1993, S. Courtemanche and S. Shaffer of this office conducted a special l safety inspection at your Camp Hill, Pennsylvania facility of activities authorized by an NRC l general license. The inspection involved an examination of your licensed activities as they relate to radiation safety and to compliance with the Commission's regulations and the conditions of the general license. The inspection consisted of observations by the inspector, interviews with j personnel, and a selective examination of representative recortis. The findings of the inspection  !

were discussed with Mr. Fred M. Gillespie, Jr. and other members of the staff at the Camp Hill, l Pennsylvania facility along with Mr. Dwayne Jones of your staff at the conclusion of the inspection.

The inspection resulted in the identification of three apparent violations. A letter containing a l Notice of Violation was issued to the management of your Camp Hill, Pennsylvania facility addressing the issues at that facility. One of the issues, however, involved management's approval of an unauthorized activity after concerns were brought to management's attention by workers. Specifically, on May 19, 1993, workers were directed to repair / replace a mylar window on an Lippke System Model MV-FM gauge. After removing the source housing, the i workers notified management that there was a label affixed to the housing that stated that only licensed personnel could remove the source. shielding or source containment housing, Management apparently considered the information provided by the workers and instructed them to remove the housing despite the prohibition on the label. We are concerned that management did not stop the work after the issue was brought to their attention.
