NRC INSPECTION i
Dear Mr. Mikowski:
This refers to a reciprocity inspection conducted on April 28,1999, with continuing NRC review through May 11,1999, at the Lake Side Veterans Administration Hospital located in Chicago, Illinois. The purpose of the inspection was to determine whether activities authorized by 10 CFR 150.20 were conducted safely and in accordance with NRC requirements. At the conclusion of the site inspection, preliminary findings were c'iscussed with a member of your staff. The continuing NRC review was performed to further review the transfer of licensed material and other regulatory issues.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commission's rules and regulations and with the conditions of your license. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, independent measurements and observation of activities in progress.
No violations of NRC requirements were identified during the course of this inspection.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter will be placed in the NRC Public Document Room.
