Dear Mr. Taylor:

This letter is to notify you of our actions in response to the above listed reference

'l' which requests that Paul-Munroe investigate the cause of failure of six ITT Neodyn pressure switches used in the assembly of Valve Operator Systems manufactured by Paul-Munroe to determine if these failures are reportable under the provisions of 10CFR21.

We have been in contact with Mr.

Ed Baker and Mr. Kamalakar Naidu of the NRC relative to this issue since 25 March 1987.

Reference '2' adds solenoid valves, servo amplifiers and potentiometers to the list of components from these systems which were found to be inoperable when the Valve Operator units were installed on the Pressure Operated Relief Valves in the plant af ter having been in storage for two years.

Paul-Munroe technicians have been on site to supervise the installation and check out of the Valve Operator Systems.

The technicians have reported that the systems were installed without their protective enclosures and that they were dirty and wet from oil and water.

The hydraulic fluid within the systems was found to be heavily dirt contaminated.

During check out, the components which were found to be inoperable were replaced.

At completion of check out, the Valve Operators were tested and deemed to be in operating condition.

Mr. James Taylor U.S. Nuclear Regulatory Commission 15 April 1987 Page Two Three pressure switches and two servo amplifiers have been returned to Paul-Munroe from the South Texas Project for evaluation and this effort is currently ongoing.

The pressure switches were taken to the manufacturer's facility by the undersigned and Mr. Kamalakar Naidu of the NRC for test and evaluation.

It is expectea that the evaluation process will take approximately thirty days.

Preliminary findings indicate that the pressure switches failed due to particulate contaminates found on the diaphragms of the switch interface to the hydraulic fluid medium.

The servo amplifiers will be returned to the manufacturer for analysis.

In addition, Paul-Munroe has now received four Valve Operators for the Pressure Operated Relief Valves from the South Texas site to be completely overhauled in accordance with the Paul-Munroe recommendations for five year service.

Our initial inspection of these units indicated that two of the servo potentiometers wre mechanically damaged.

These studies and the valve operator overhaul are expected to be complete in approximately thirty days at which time Paul-Munroe will file a complete and final report.

t We have received a nonconformance report from the South Texas site which indicates that 6 of 8 pressure switches on the PMi Eleccro-Hydraulic operators have failed.

'! hose failures are described as internal rupturing. We believe your service personnel may be aware of some of the details of these failures.

We have been advised that these failures have been reported to the NRC as potential difficiency under 50.55E'.

CCI request that Paul-Munroe review these failures to determine if they are reportable under 10 CPR 21. We request that you advise us the results of your investigation by 3/26/87.

Thepotentiometer(valveposition transducerD provides volve position feedback to the remotely located serve-emp1Lfter.

The servo amplifier compares the feedback signal with the incoming 4 to to milliamp comand signal to the servo.ampl' fier from QOPS and generates a signal to centrol the two solenoid valves A1M5 PY.7411A and A1N5.PY.74118 until the valve position corresponds with the incoming coesnd signal.

In swanary failure of any of these components affects the operation and reliabi1Ity of the p0RV.-

The PORV's and associated pressure costrels provide the capability for 4 controlled plant coeldown when the Turbine B.vcass System (T85)Syst is not 1

available.

During abnemal operation model of the Main Steam the PORV's are opened to the atmosphere to remove decay heat from the steam senerators in the event the T85 is not available.

In the event the PORV's cannot relieve the steau f a the steen ponerators, the main steam ssfety relief valves progressively open to re teve the steam to the atmosphere via vent stacks without over-pressurtzing the steam generators.

This incident is potentially reportable under 10CFA50.55(e). Further evaluation is necessary.
