Gentlemen:
Subject: NOTICE OF VIOLATION (NRC Inspection Report No. 50-29/86-09)
This refers to the NRC safety inspection conducted on June 26 - July 2, 1986,
at the Yankee Nuclear Power Station, Rowe, Massachusetts. The inspection report
was sent to you on July 11, 1986.
The purpose of the inspection was to review
the circumstances associated with a deficiency at the facility which occurred
in 1985 and which was identified by your staff in June 1986.
This error resulteo
in two violations of technical specification requirements. On July 22, 1986, an
enforcement conference was held with you and members of your staff to discuss
the violations, their causes, and your corrective actions.
The violations involve the installation of undersized trip coils in circuit
.
breakers for motor operated valves located in the reactor vessel head vent pa'h,
the pressurizer steam space vent path, and an alternate emergency feedwater
system flow path. The undersized trip coils would have caused the valves to
trip prenaturely during design basis operation. These violations demonstrate
the importance of: (1) clear description and specification in design documents
and purchase orders used to procure safety-related materials; (?) adequate
attention to detail during inspection of procured parts prior to their use in
the facility; and (3) effective preparation and review of test procedures to
ensure that post-installation testing of parts actually verifies the ability of
the parts to perform their intended function under all operating conditions.
In this case, the violations could have been prevented if the design documents
and resulting purchase order had been more specific, the receipt inspector had
been more inquisitive, or the post-installation test procedure had been more
thorough.
In accordance with the " General Statement of Policy and Procedure for NRC
Enforcement Actions," 10 CFR Part 2, Appendix C (1986) (Enforcement Policy),
the violations described in the enclosed Notice have been classified in
the aggregate as a S" verity Level III problem. A civil penalty is considered
for a Severity Leval III violation or problem. However, after consultation with
the Directo , Office of Inspection and Enforcement, I have decided that a civil
penalty will not be proposed in this case because: (1) the error was identified
by your staff and promptly reported to the NRC; (2) your corrective actions were
prompt and comprehensive; and (3) you have demonstrated prior good performance

in the areas of operations, maintenance and surveillance as evidenced by
Category I ratings in these areas during the last five Systematic Assessments
of Licensee Performance.

You are required to respond to'this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your rerponse,
you should document the specific actions taken and any additional actions you
plan to prevent recurrence., After reviewing your response to this Notice,

including your proposed corrective actions,_the NRC will determine whether
further NRC enforcement action is necessary to ensure compliance with NRC
regulating requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules- of Practice," Part 2, Title 10,
Code of Federal Regulations, a copy of this letter and its enclosure will
