Millstone 3 Routine Inspection 50-423/86-21 (6/24/86-8/11/86)

This transmits the report of the above subject inspection by Messrs. J. Shediosky

and F. Casella of this office.

The inspection findings have been discussed with

Mr. J. Crockett of your staff.

No violations were identified during this inspection.

No reply to this letter is

required.

Our inspectors noted a conservative attitude of key management personnel in ful-

filling the intent of safety and technical specification requirements.

This atti-

tude was repeatedly demonstrated during outage management and PORC meetings.

How-

ever, while the safety significance of the excessive Loop 2 Hot Leg RTO response

time was being deliberated on July 11, we believe that it would have been more

prudent to have tripped the 6 bistables associated with this instrument as soon

as the safety analyses were in question.

In this case, the bistables were tripped

after the day's deliberations were complete and the vendor recommendation was re-

ceived.

We noted the competent and timely response of operations personnel to the loss of

all fourth point feed heater drain pumps on July 21, preventing a total loss of

feedwater and a challenge to the reactor protective systems.

Good communications

and definite lines of authority were evident throughout this incident, which oc-

curred during a shift turnover.
