Areas Inspected:

A special, unannounced inspection was conducted concerning

alleged violations of NRC requirements.

Areas inspected included facilities,

instrumentation, and equipment; security of materials; confirmatory

measurements; personnel monitoring, and operational safety.

Results:

The allegation was partially substantiated and one violation was

identified (personnel dosimetry records were not available for inspection,

section 7).

Purpose for Special Inspectico

On December 18, 1985, the Region IV staff received a call from an

individual alleging radiographic work was being performed in Tulsa,

Oklahoma, by QEST, Inc. without proper licensing.

The alleger believed

QEST, Inc. had obtained a Kansas license to avoid delays encountered in

obtaining a NRC license.

Management /0rganization

On March.7, 1986, the NRC inspector visited Word Industries, Inc. where it

had been reported that QEST, Inc. had been performing radiography.

Upon

locating QEST personnel, the NRC inspector determined that QEST, Inc. was

an Oklahoma based organization in the process of incorporating in the

state of Oklahoma with* an address at 4141 South Galveston, Tulsa,

Oklahoma. The corporate officers for QEST, Inc. are Mr. Jim Wilson,

president and Mr. Dan Potter, vice president.

The NRC inspector also

learned that QEST, Inc. had applied for an NRC license from Region IV.

Upon further discussion with QEST personnel, the NRC inspector determined

that QEST was operating in the state of Oklahoma under reciprocity of a

Kansas license held by an organization called QEST Company based in

Wichita, Kansas.

Note, QEST, Inc. and QEST Company are two separate

entities.

(Dan Potter, vice president /QEST, Inc. is a brother of R. J.

Potter, RSO/QEST Company).

Upon returning to the regional office, discussions were held with regional

management and regional counsel and it was determined that the QEST, Inc.

was a legal entity in the state of Oklahoma and should be working under

an NRC license. The issue was discussed with the QEST Company and the

,

NRC expressed concern over the ability of the Kansas licensee to

'

effectively control the operation in Oklahoma.

The QEST Company agreed

to curtail their radiography work in Oklahoma pending the NRC review and

awarding a license to the QEST, Inc. group in Oklahoma.

Region IV

received confirmation of their course of action by letter from the QEST

Company on March 12, 1986.

The allegation was partially substantiated in that the QEST, Inc. was

operating under authority of an agreement state license while waiting for

their NRC license application to be processed by Region IV.

No violations or deviations were identified.

4.

Facilities, Instrumentation, and Equipment

During a tour of the area of QEST, Inc. activities, the NRC inspector

inspected the fixed radiography facility at Word Industries.

QEST, Inc.

was performing the major portion of their radiography at the Word

facility.

The inspection determined the access and exit controls on the

alarm system were functioning properly and the visible and audible warning

signals were also functioning properly.

The NRC inspector determined the

equipment was adequate and operating properly. The survey meters were

adequate, functioning properly, and had been calibrated.

No violations or deviations were identified.

Security of Materials

The NRC inspector determined that the radiographic sources were secured

adequately in steel storage lockers.

Only QEST, Inc. personnel had keys

to these storage lockers.

No violations or deviations were identified.

6.

Confirmatory Measurements

The NRC inspector performed surveys of the radiographic sources and

facility to determine compliance with 10 CFR 34.21 and 20.105,

respectively. QEST, Inc. survey instrumentation compared favorably with

the NRC inspector's survey instrument during these surveys.

No violations or deviations were identified.

7.

Personnel Monitoring

The NRC inspector determined the personnel dosimetry which includes TLD

badges and pocket dosimeters to be adequate. A review of the dosimetry

records revealed that for the period of December 15, 1985 to January 14,

1986, these records were missing and were not available for inspection by

the NRC on March 7, 1986. This was identified by the NRC inspector as a

violation of 10 CFR 34.33.

Operational Safety

The NRC inspector reviewed the operating procedures of QEST Company and

determined that they were adequate.
