DUKE POWER COMPANY RESPONSE TO VIOLATION j
413/86-45-02 AND 414/86-48-01 i
1 Technical Specification 4.7.4.a requires that, at least two Nuclear Service Water loops shall be demonstrated operable at least once per 31 days by verifying that each valve (manual, power operated, or automatic) servicing safety related equipment that is not locked, sealed, or otherwise secured in position is in its correct position.
. Contrary to the above, at least two nuclear service water loops were not demonstrated operable by verifying that each valve is in its correct position in that manual valves IRN-C21. -C22, 1RN-21, -22, -23, -25, which are in the seal water supply lines to the nuclear service water pumps were not included in the monthly valve verification surveillance check lists.
This was identified by the inspector on October 30, 1986. This violation is applicable to Units 1 and 2.
RESPONSE
(1) Admission or Denial of Violation Duke Power Company admits the violation.
(2) Reason for Violation Technical Specification 4.7.4.a requires "At least once per 31 days... each valve (manual, power-operated, or automatic) servicing safety related equipment that is not locked, sealed, or otherwise secured in position is in its correct position". OP/0/A/6400/06C, " Nuclear Service Water System", did j
not require the affected valves to be locked, and PT/1(2)/A/4400/o2C, " Nuclear Service Water Valve Verification", did not require the affected valves to be verified in the correct position.
But, Operations Management Procedure 2-19, l
Rounds Sheets, requires verification of pump seal, lubrication, and cooling water flows to operating pumps twice each day.
(Cooling water isolation valves are interlocked to open on pump start and close on pump shutdown.)
Verification of actual flow rates should exceed the requirement to verify valve alignments. Normal station operational needs have assured that inservice pumps are frequently rotated. A review of plant records for the last four months of 1986 confirmed this.
This review also showed that operators routinely verify lubrication and seal water flow rates to non-operating pumps. Adequate indication exists in the control room to warn the operator in the event that a Nuclear Service Water pump is started without I
adequate cooling water supply.
(3) Corrective Action Taken and Results Achieved Valve position has been verified and locks added to secure the valves in their corrective position.
(4) Corrective Action Taken to Avoid Further Violations j
Operations Procedure (OP/0/A/6400/06C) " Nuclear Service Water System" has been revised to require locking of the affected valves.
