o SPECIAL REPORT 85-05 REVISION 3 SEQUOYAH NUCLEAR PLANT UNIT 2 This revision only corrects a typographical error in a date in the second paragraph.
On July 23, 1985, at 0705 CST, it was discovered that fire door A-9 to residual heat removal pump room 2B at Auxiliary Building elevation 653 would not close properly. At that time, units 1 and 2 were operating in mode 1 at 100-percent power (2235 psig and 578 degrees F).
An hourly fire watch was already being conducted for the subject area due to Appendix R considerations and was continued.
An investigation showed that due to high usage the door hinge support plate inside the door was broken. The door was temporarily repaired on October 27, 1985, and returned to service.
It was specified in our original report that the door was scheduled for permanent replacement by December 18, 1985. However, due to procurement difficulties, revision 1 to this report revised the schedule to May 30, 1986.
Door A-9 was received onsite May 13, 1986, and the proper documentation was provided by the supplier on May 22, 1986.
During an attempt to install the new door in the frame, it was discovered that the. door hinge cutouts were improperly located by the manufacturer and would not align with the frame /
hinges. Because the door is Underwriters Laboratories (UL) labeled, only UL licensed repair / manufacturing facilities can modify the door. For this reason, the door must be returned to the manufacturer for correction of the hinge portion. Allowing sufficient time for return, repair, receipt, and installation, we now expect to have the new door functional by August 30, 1986. The existing door is currently functional.
This report is required per the special report requirements of Technical Specification 3.7.12.
There was no effect on public health or safety.
