DUKE POWER COMPANY RESPONSE TO VIOLATION 414/86-04-01 10 CFR 50, Appendix B, Criterion V, as implemented by paragraph 17.2.5 of DPC Topical Report Duke-1, requires that activities affecting quality shall be prescribed by drawings or procedures and that activities be accomplished in accordance with these drawings or procedures.
For the Containment Ventilation Condensate Drain Tank Support, drawing CN-1222-4 and referenced sketch CN-1222-4Aa require two nuts for each anchor bolt. Paragraph 4.2.2 of Construction procedure 117 requires that nuts on sliding feet anchor bolts be finger tight and that nuts on non-sliding bolts be finger tight plus i turn.
Contrary to the above, on February 3, 1987, installed equipment anchor bolts did not meet drawing and procedure requirements in that, for the Containment Ventilation Condensate Drain Tank Support, two anchor bolt primary nuts were backed off approximately two inches.
In addition, jam nuts were missing on six anchor bolts and jam nuts were backed off approximately two inches on six other anchor bolts.
RESPONSE
(1) Admission or Denial of Violation Duke Power Company admits the violation.
(2) Reason for Violation No evidence can be found to account for this condition.
Construction Department records indicate the item was installed and inspected on April 13, 1978. Construction records indicate that this item was never worked on again after initial installation. Nuclear Production records indicate that this item has never been worked on.
(3) Corrective Action Taken and Results Achieved Missing nuts and washers were installed and tightened by the appropriate Design drawing. Work was completed by 1915 on February 6, 1987. The corresponding installation on Unit I was examined and found to be per Design drawings.
(4) Corrective Action Taken to Avoid Further Violations No further action is planned since this is considered to be an isolation case.

(5) Date of Full Compliance Full compliance was achieved on February 6, 1987.
