Gentlemen:

This refers to the meeting held with you and members of your staff on

October 7,1988, .at your Portland office.

Subjects discussed during the

meeting are described in the enclosed meeting report. We concluded that the

events and forced outage activities in mid-September 1988 identified a new

concern regarding the adequacy of your locked valve program, and significantly

re-emphasized a number of concerns which have been discussed with you and your

staff oli previous occasions.

Tilese concerns include the inadequacy of

procedures / instructions and inadequate pre-work briefings, the failure of

personnel to follow procedures, and the apparent problem of people not

thinking sufficiently about what they are doing and proceeding in the face of

j

'mcertainty.

It is my understanding that you are fully committed to

significantly improving performance in these areas.

Further, it'is my

understanding that you will promptly modify your locked valve program to

require use of breakable seals on the appropriate valve chains, such that any

unauthorized change of valve positions can be readily detected.

If you have any questions concerning this management meet.ng report, we will

be pleased to discuss them with you.

In accordance with 10 CFR 2.790(a), a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.
