INADEQUATE PROCEDURES FOR CONTROL OF RADIOACTIVITY

Technical Specification, paragraph

6.8.1.a.

requires that procedures

shall he established, implemented, and maintained for

activities

recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February

1978. Paragraph 7 of Appendix A to Regulatory Guide 1.33 delineates

procedures for the control of radioactivity.

Contrary to the above, it was found on February 9, 1987, that there were

no procedures in effect directing personnel exiting through portal

radiation monitors as to what should be done if a portal monitor

alarmed.

.

This is a Severity Level IV violation.

REASON FOR VIOLATION

The subject violation was reviewed for determination of the cause.

The

results of the review identified that proper action for portal monitor

alarm was addressed in General Employee Training, but not specifically

addressed

in

the

Radiological Program and Security procedures.

Therefore, the cause was determined to be insufficient information

regarding the portal monitor alarm in the Radiological Program and

Security procedures.

CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED

Radiation Protection has revised RPP-0043, " Personnel Frisking",

to

include the following statement.

"When passing thru a Portal Monitor, if the alarm sounds, reset the

monitor and pass thru again.

If second alarm sounds, notify

Radiation Protection.

Personnel alarming Portal Monitors after

resetting must be released by Radiation Protection."

Security has revised SPI-4 " Security Position Instruction" as follows:

1.

If an Officer is on duty in the exit area and the portal

monitor alarms when an individual exits, the individual will be

stopped, instructed to reset the portal monitor and pass

,

through again.

If the monitor alarms a second time, Radiation

Protection will be contacted to perform a contamination survey

If an Officer is not on duty in the exit area and the portal

monitor alarms, the Access Control Officer will lock the

  • -

turnstile and the same procedure in paragraph one will be

followed.

A handout was given to personnel exiting the plant and also

personnel at the Main Administration Building personnel exit and

Gate 9.

This memo was also included in the Safety Bulletin.

A printout of personnel exiting at the same time as the Condition

Report (CR)

initiator was obtained and a comparison of Radiation

Work Permit (RWP) entries was accomplished. It was determined that

personnel entering and exiting at approximately the same time as the

CR initiator had not worked in areas of significant radiological

hazards.

An additional alarm has been attached to the portal monitors which

is much louder than the alarms furnished with the instrument. This

should ensure personnel hear the alarm during rush periods.

Implementation and enforcement of

revised

procedures

has

been

accomplished.

CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS

RBNP-024, " Radiation Protection Plan",

will be revised by 5/29/87 to

include the same statement as RPP-0043.

DATE WHEN FULL COMPLIANCE WILL BE ACHIEVED

Full compliance will be achieved as of 5/29/87.
