5. It is apparent that at this point the current license l applications including Rod's were resented to the Training Coordinator for review. Mr had not signed his application at this time. All of the then curren license applications were then signed on Line 17c of NRC Form 398 by the Tr Coordinator on 12/8/83. He stated that he knew Mr had not signed the application and was aware that he was current in Requalification Training. Mr. Nelson verbally the responsible clerical personnel not to send Mr. t lication until these deficiencies werg wucced.

6. The license applications were then sent to Cook Plant '

Managerial where W. G. Snith, Jr., signed on Line 17c on 12-10-83. The applications were then forwarded to AEP where M. P. Alexich signed on the same line on 12-16-83.

7. The applications were then returned to DC Cook Training to be sent to the NRC. They were sent to the NRC on 12-20-83 and they were received by the NBC on 12-23-83.

8. During processing by the NRC it was noted that Mr had not  !

signed his application and it was therefore re im with a letter of explanation requesting applicant's signature on Line 17a, Page 2, NRC form 398. This letter of explanation had cc: for Plant Manager and Training Supervisor. The letter was received by Cook Managerial on 1-3-84 and forwarded to Personnel where it was received on 1-4-84.

9. E en Mr. bived the above request from the NRC, and his license app tion with signatures fran Training, Man =y=mt, and corporate levels in place, his opinion was the .

FCous W. A. NICHOIS TC W. G. SMITH I ccmpared the Licensed Operator Peplacement Training Program described in Section 12 of the DC Cbok FSAR with our present program to ensure we meet our FSAR ccumitrents as related to IE Notice 84-11.  ;

l The FSAR ccumits us to provide formal classroca lectures, training at a research reactor, on-the-job training including simulator training, an audit exam prior to the NRC exam, and review training. All of these ccrmitments are included in our present program in nore depth than )

described in the FSAR. We do not have any deficiencies regarding the Operator License Training Program cocpared to FSAR crnmitments.

I did cone across one item of concern related to STA's that I want to bring to your attention. Section 12 of the FSAR states that:

" Prior to selection to the program, the SIA candidates shall have been working at the Donald C. Cook Nuclear Plant in a responsible capacity for a minimum of one (1) year and shall possess a Bachelor of Science Degree in an engineering related field. In addition, each candidate will meet the one (1) year of professional level nuclear power plant experience requirement outlined in Draft Standard ANS 3.1 dated Decenber 17, 1979."

'Ihis statement may be a potential problem area in that we are hiring STA l candidates without DC Cbok Plant experience.

that the 33 individuals above also had other errors and discrepancies /-

deficiencies in their RO/SRO applications to the NRC, including, (1) incorrect course durations, (2) training courses listed on the applications which are not documented in training files, and (3) training courses listed on the applications in which applicants admittedly did not participate -

Additionally - investigation disclosed that 9 other applicants (these Og 9 applications did not have the entry Cold License Operator Qual. Card listed) had errors, discrepancies / deficiencies noted in their applica-
