Dear Sir:

his letter refers to the violation cited in 'the subject inspection report which states:

"During the Nuclear Regulatory Commission (NRC) inspection conducted on January 11 - February 10, 1987, a violation of NRC requirements was identified. The violation involved failure to follow procedures during replacement and cleaning of diesel generator lube oil strainer elements. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is listed below:

Technical Specification 6.8.1.a requires that written procedures shall be established and implemented covering the activities referenced in Appendix "A" of Regulatory Guide (RG) 1.33, Revision 2, 1978. Section 9 of RG 1.33, Appendix "A", requires that procedures for the control of maintenance. repair and replacement should be established. System Operating P. c. :ec' 're PNP-O-SOP 38.2, Changing and Cleaning Lube oil Strainer on DieseA Generator, implements this requirement and requires that the lube oil strainer elements part number be verified.

i Contrary to the above, on February 2, 1987, it was discovered that PNP-O-SOP 38.2 had not been fully implemented in that diesel generator

. lube oil strainer elements were verified to have improper part numbers.

Subsequent review by the licensee identified a total of four out of five diesel generators had incorrect lube oil strainer elements.

Reason for Violation 1.

We violations on the IB and 2C diesel generators (D/Gs) were caused by procedural. inadequacy in that the strainers were changed prior to the revision to FNP-O-SOP-38.2 which added a requirement for strainer i '

element identification.

2.

We violations on the 1-2A and 1C D/Gs were caused by personnel error

in that the individuals. failed to follow procedure and ensure that the correct strainer element was installed in the appropriate D/G as required by INP-O-SOP-38.2.

l Corrective Action Taken and Results Achieved 1.

All D/G lube oil strainers were checked and the correct elements installed.

2.

Correspondence was requested and received from Colt Industries indicating that use of the incorrect strainers on the 1-2A and 1B D/Gs should not have caused any damage. Also, correspondence was received on the 1C and 2C DAs stating that the use of a 40 micron strainer in the lube oil system instead of 74 micron should not create a problem, but that the frequency of strainer cleaning would probably increase.

l Corrective Steps Taken to Avoid Further violation l'

Personnel who last signed for having installed the correct strainer element on the 1-2A and 1C D/Gs have been disciplined for failure to follow procedure. The strainer elements on the other D/Gs were j

apparently changed prior to the procedure being revised to include l

element identification.

l 2.

FNP-O-SOP-38.2 has been revised to make it clear as to where the strainer element numbers are located on the strainer elements.

3.

This incident has been covered with all on-shift Operations personnel at crew briefings stressing that personnel ensure each individual l

fully performs and understands any procedure steps he signs.
