ENCLOSURE NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-327 and 50-328 Sequoyah License Nos. DPR-77 and DPR-79 During the Nuclear Regulatory Comission (NRC) inspection conducted on February 24 - 28, 1986, a violation of NRC requirements was identified.
The violation involved inspection and installation of concrete expansion achor bolts pertaining to safety-related piping systems.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Appendix B, Criterion V of 10 CFR 50, requires that activities affecting quality shall be prescribed by documented instructions and procedures, and shall be accomplished in accordance with t bese instructions and procedures.
General Construction Specification G-32 requires that the installers are to be instructed to tighten the bolts between 1/8 and 1/4 turn after the bolt head comes into contact with the attache nt or baseplate.
Contrary to the above, between Februar!/ 24-28, 1986, activities affecting quality had not been accomplished in accordance with documented instructions and procedures in that the licensee's QC inspectors failed to detect the anchor bolt deficiencies for nine pipe supports and one pump support plat-form baseplate during their inspections.
The deficiencies were the gaps between the bolt-heads and the baseplates.
This is Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further viola-tions, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
