This letter transmits the results of the routine inspection conducted by Messrs.
A. Luptak and R. Plasse of this office on August 8, 1938 - October 5, 1988, e
the J. A. FitzPatrick Nuclear Power Plant, Scriba, New York.
Mr. Luptak
discussed the inspection findings with you and other members of your staff at
the conclusion of the inspection.
Based on this inspection, an apparent violation of NRC requiruents has been
identified and is described in the enclosed Notice of Violation.
The violation
involved several examples of failure to follow procedures. These involved the
use of a drawing which was not in the drawing control system and charging of a
hydraulic control unit accumulator which resulted in inadvertent control rod
insertion during refueling operations. You are required to respond to this
letter; in doing so pl(ase follow the instructions in Appendix A of this letter.
The response directed by this letter and the accompanying Notica of Violation is
not subject to the clearance procedures of the Office of Management and Budget
as required by the Paperwork R' action Act of 1980, PL 96-511.
