Murphy Vice President and Manager of Operations RD 5, Box 169 Ferry Road Brattleboro, Vermont 05301 Gentlemen: Subject: NRC Region I Inspection Report No. 50-271/88-14 This refers tu the routine resident safety inspection conducted by Mr. G. Grant of this office on August 23 - October 10, 1988, at the Vermont Yankee Nuclear Power Station, Vernon, Vermont. The inspection consisted of observation of activities, interviews with personnel and document reviews. The results of the inspection are described in the NRC Region I Inspection Report enclosed with this letter and were discussed with Mr. J. Pelletier of your staff af ter the conclusion of the inspection. Based on the results of this inspection, it appears that some of your activ- ities were not conducted in full compliance with NRC requirements, as set forth in the Notice of Violation enclosed herewith as Appendix A. These violations concern a failure to meet the limiting condition for operation in Technical Specification 3.13.G.2 concerning firewatch requirements when the recirculation motor generator set foam system is inoperable, and failure to meet 10 CFR Part 50, Appendix B, Critorion III, requirements that design control measures shall provide for verifying or checking the adequacy of design. Additionally, another violation was identified concerning a repeated failure to meet the limiting condition for operation of Technical Specification 3.9. A.1 and Table 3.9.1 requirement for obtaining 24-hour grab samples of the service water system when the ef fluent radiation monitor was inoperable. These violations have been categorized by severity level in accordance with the "General Statement of Policy and Procedure for Enforcement Actions", 10 CFR Part 2, Appendix C. You are required to respond to this letter and, in so doing, should follow the instructions in Appendix A. In addition to the corrective actions taken or planned as a result of the violations issued in Appendix A, your response should include your coordinated approach 1o resolving all fire protection program issues addressed in this report.
