DAVIS-BESSE INSPECTION REPORT 50-346/99005(DRS)
Dear Mr. Campbell;

On April 9,1999, the NRC completed a routine on-line maintenance risk assessment
inspection at your Davis-Besse Nuclear Power Station. The enclosed report summarizes the
inspection results.
In November 1998, Davis-Besse experienced a manual reactor trip that was due, in part, to
authorized maintenance activities on components associated with a protected train during
plant operation. As a result of that reactor trip, we questioned the effectiveness of your work
control processes and the risk maintenance matrix that you used when evaluating and
approving on-line maintenance work activities. The purpose of this inspection was to examine
your on-line maintenance risk assessment program recently implemented in response to some
of these maintenance work control weaknesses. The inspection also evaluated risk
assessments performed to support engineering issues, such as, performing system outages
- during a plant shutdown versus on-line, temporary modifications; and planned outages.

Although still in the initial implementation stages, the new on-line maintenance risk program
was a significant improvement from wb-0 the plant had used in the past and appeared to be
working well. The process had the necessary steps to demonstrate that emergent activities
were adequately planned and controlled to avoid causing initiating events to occur or affect the
functional capability of mitigating systems. In particular, we noted risk assessments that were
thorough, of good quality, and resulted in keeping the plant's aggregate risk at a minimum.
The risk categorization of surveillance tests and increasing the initiating evert frequencies to

account for surveillance testing in your analyses were considered strengths. No violations of
NRC requirements were identified during this inspection.
