SUBJECT: NRC INSPECTION REPORT NO. 50-361/99-03; 50-362/99-03
Dear Mr. Ray:
This refers to the special inspection conducted on February 5 through March 12,1999, at the
San Onofre Nuclear Generating Station, Units 2 and 3, facility, in response to a loss of
shutdown cooling event that occurred in Unit 2 on February 1,1999. The enclosed report
presents the results of this inspection.
Basad on the results of this inspection, the NRC has determined that one violation of NRC
requirements occurred. This violation is being treated as a noncited violation (NCV), consistent
with Appendix C of the NRC Enforcement Policy. The NCV is described in the subject

inspection report. If you contest the violation or severity level of the NCV, you should provide a
response within 30 days of the date of this inspection report, with the basis for your denial, to
the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington,
DC 20555-0001, with copies to the Regional Administrator, Region IV, the NRC Resident
inspector at the facility that is the subject of this report, and the Director, Office of Enforcement,
United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
Once the loss of shutdown cooling event occurred, the response of your Operations staff to
mitigate the effects of the loss was excellent and your defense-in-depth program successfully
ensured all safety functions were maintained. However, the event was the direct result of your
staff failing to recognize that the risk, which was associated with racking out a safety bus feeder
breAct, increased substantially when breaker interference was identified. More careful
planing and control was warranted, when the risk increased. Two significant opportunities to
intervene and prevent the loss of shutdown cooling event were missed. We agree with the
conclusions of your Level 1 event report, which documents your investigation of the causes of
the event. In addition, we find that your p!anned and completed corrective actions for the
violation, and for other weaknesses identified as a result of the event, were directed toward
improvement of your programs, procedures, and safety culture.
