INSPECTION N0. 50-267/87-04

Enclosed is the report of the team inspection conducted by Mr. G. T. Hubbard

and other NRC representatives en January 26-30, 1987, at the Fort St. Vrain

Nuclear Generating Station, Platteville, Colorado, of activities authorized by

NRC License DPR-34. The team's findings were discussed with you and members of

your staff at the conclusion of the inspection. The inspection reviewed your

implementation of a program for establishing and maintaining the qualification

of electric equipment within the scope of 10 CFR 50.49. Within these areas,

the inspection consisted of examination of selected prccedures and records,

interviews with personnel, and observations by the inspectors.

The inspection determined that you have implemented a program to meet the

requirements of 10 CFR 50.49 except for the deficiencies identified in the

enclosed inspection report. The two deficiencies, summarized in Appendix A,

are classified as Potential Enforcement / Unresolved Items and will be referred

to the Region IV Office for further action. One deficiency involved failure

to document qualification of first generation Rockbestos coaxial cable and the

other deficiency involved failure to establish nualification of certain Raychem

splice configurations installed in the plant.

Five additional concerns are classified as Open Items.

Three of these items

require action by you and will be reviewed during a future NRC inspection.

The remaining two items involve Region IV review of the implementation of your

EQ maintenance and procurement programs. Details of the deficiencies and

concerns are discussed in the inspection report.

Your corrective actions regarding the deficiencies and concerns identified in

the enclosed inspection report should not be delayed pending either a future

NRC inspection or further actier by the NRC regional office.

In addition to correcting the deficiencies and concerns identified during the

inspection, you should also complete the outstanding EQ activities identified

during the inspection entrance meeting. These items include, but are not

limited to, completion of six qualification binders, verification of phone

conversation records, and completion of plant ir.stallation and maintenance

activities.

APPENDIX A

Potential Enforcement / Unresolved Items

As a result of the equipment qualification inspection conducted on January 76-30,

1987, the following items will be referred to NRC Region IV as Potential Enforce-

ment / Unresolved Items (paragraph references are to detailed portions of the

inspection report).

1.

Contrary to the requirenents of paragraph (f) and (k) of 10 CFR 50.49 and

Section 5.2.2 of the D0R Guidelines, at the start of the inspection, Public

Service Company of Colorado (PSC) failed to adequately establish qualifi-

cation of Rockbestos coaxial cable installed at Fort St. Vrain (FSV).

Specifically PSC failed to establish similarity between the first generation

cable installed in the plant and the later generation cables which were

tested in the qualification reports included in the qualification binders.

2.

Contrary to the requirements of paragraph (f) of 10 CFR 50.49, PSC did not

adequately establish qualification of certain Raychem splices installed

in FSV. PSC had not documented that certain installed splices were iden-

tical to the qualification test specimens used to establish qualification

in the binders. Specifically, the inspectors observed splices installed

in plant equipment where the shin sleeve extended past the splice sealing

sleeve and there was no seal between the splice sealing sleeve and the

cable insulation.
