Dear Mr. Jenkins:

This is to confirm the permit exception that was previously reported to you by telephone on May 27, 1986.

On May 21, 1986, operations personnel found that the Package Boiler "A" blowdown tank, discharge number 091, had drained. It was subsequently determined that an improperly seated valve had been the cause. Due to this unplanned drafilage, no effluent sample was obtained as required by our NPDES permit. Operations personnel verified that all other associated valves were closed, and as corrective action, a formal work request was submitted for repair of the valve. '
